The health care sector in the US continues to evolve; several health reforms aim at addressing accessibility, affordability, and quality (Kim, Farmer, & Porter, 2013). The US health system has a long way to achieving its intended objective due to enormous challenges in the process of expanding care services. Like other countries, the US does not have a universal system of care delivery. Instead, care delivery is fragmented making it difficult to coordinate health care services effectively (Fireman, Bartlett, &Selby, 2010). In the US, health care is delivered at different levels with different agencies given the mandate to compete with other service providers within similar locations. This strategy aims at reducing the cost of health care. Health Maintenance Organization and Accountable Care Organizations are two health care systems commonly used in the US to deliver care. Accordingly, this paper analyzes two health care delivery systems in the US, Health Maintenance Organization and Accountable Care Organizations examining the similarities and differences among them. The paper will present the mission and objectives besides recommending the principles that can be applied to improve the health care system.
Health Maintenance Organizations
Health Maintenance Organizations (HMOs) are private entities contracted by the government to improve accessibility to care. Health Maintenance Organizations offer insurance coverage for patients under the Medicare according to specified age and cost. HMOs use networks of health practitioners located within one geographical area. They develop standards of operations inducing medication and services that can be offered by the practitioners. For one to subscribe for membership in a HMO, it entails paying monthly premiums as an individual or through the organization (Tilburt et al., 2013).
Members can also be automatically registered within the network by their employers who pay the monthly premiums. HMOs provide restricted clinical services; they only pay for authorized services, prescriptions, and treatment, which are appropriate. They analyze and review medically important procedures before one’s payment is authorized (Tilburt et al., 2013). HMO aim at minimizing unnecessary, expensive medical procedures, and promoting wellness through preventive services compared to other service providers who focus on curative and restorative care services. Some of their preventive services include screening programs, health education, annual physical examination, and early illness detection initiatives (Tilburt et al., 2013).
The Future Mission of Health Maintenance Organizations
In the future, HMOs should focus on reaching out to the under-served population. Part of their long-term strategies should include offering patient-centered care where patient cultural beliefs and practices form part of holistic care to minimize existing health disparities (Fireman, Bartlett, &Selby, 2010). Cultural competence care results to high-quality outcomes because patient will feel comfortable expressing their concerns and physician making well –informed decisions by incorporating patient’s needs. In the future, HMOs need to adopt innovative strategies by engaging in continuous quality improvement programs through on-going research and community support programs (Fireman, Bartlett, &Selby, 2010).
