The steps to code using the CPT manual
Step 1 Review the physician’s report, the information and the process of procedure in the report
Step 2 Review the CPT Codebook focusing on the code and the index
Step 3 Determine the main terms and appropriate sub term 
Step 4 Review the Report and Documentation
Step 5 Make a Preliminary Code Selection. 
Step 6 Cross reference the code and the alphabetic index
Step 7 Verify the code and focus on the highest level of specificity
The Common Procedural Terminology (CPT) guideline are used to determine the right code level and correct codes (Brennan, McGuire & Metzler, 2016). Developing a plan of care in CPT evaluation depends on the coding activities, which helps to choose the appropriate   services or procedures.
