In this section, you should detail your comprehensive approach for how you will communicate this change and its impact, how you will keep people informed of the project’s progress, and how you will keep people informed of the project’s ongoing results.
Phase III—Evaluation Strategy: This plan for managing and measuring progress and success of your initiative should include the following:
A. discussion of the specific goals and milestones of implementation of the initiative. Lay out your goals in a clear, obtainable fashion to ensure that progress milestones are clear, reasonable, and consistent.
B. Specific definitions of measures of progress toward obtaining the intended goals and a description of the tools that you would use to measure progress. What strategies will be used to measure success, and how will adjustments be made should progress toward these goals fall short or change?
C. How do you intend to collect and analyze information to ascertain stakeholder satisfaction with the initiative? What information will be looked at, and who are the stakeholders that will be considered?

An efficient and effective communication plan will be crucial to keeping all stakeholders informed. The key stakeholders' ability to make informed decisions depends on providing the right information when needed and in the appropriate format. Therefore, the fundamental goal of this initiative is to support community health by providing holistic care services. In this case, the project's major funders are all of the shareholder groups. The Tower Healthcare Systems' caregivers and other clinical officials will also need information from the initiative. Patients in the Pennsylvania region will also need information because they will assess the project's success. As a result, communication between various parties will need to occur. The initiative will exchange information using several techniques to guarantee an efficient information flow. The plan will incorporate various communication methods, such as face-to-face conferences, teleconferences, emails, and social media (Nothhaft et al., 2021). During meetings, the plan for the initiative will be physically presented to stakeholders for feedback. At the beginning of the initiative and then regularly moving forward, these meetings will be held to provide progress reports and receive suggestions for improvements. Teleconferences will be utilized for updating interested parties who cannot attend in-person meetings. The in-person meetings and these calls will go on at the same time. Email updates will be regularly sent to all parties involved (Joshi, 2020). These updates will include specifics on the project's status, changes, and results. A project page will be made on social media so interested people may learn more about it and provide suggestions. The project page will be updated from time to time with new information.
Evaluation Strategy
Tower Healthcare Systems’ objective of this initiative is to transform lives through physical, emotional, psychological, and spiritual well-being for the community.
