The future of nursing: IOM Report
The Robert Wood Johnson Foundation (RWJF) collaborated with the Institute of Medicine to establish an initiative focusing on the future of nursing. In the joint initiative, the report highlighted RWJF recommendations for the future of nursing and transforming nursing (Sullivan, 2018). The Committee envisioned a future health care system that provided quality, affordable healthcare to diverse communities in the U.S. The Committee promoted a system that intentionally promotes wellness and disease prevention, improves health outcomes, and provides compassionate care across all lifespan (Committee on the Robert Wood Johnson Foundation Initiative on the Future of Nursing, 2011). To create the action-oriented blueprint, there were five meetings, with three being technical workshops with experts to develop information for the study. There were also three public forums that addressed critical issues, The Committee also conducted site visits for various healthcare facilities to determine how nurses function in different healthcare and education settings. The Committee welcomed the public's testimonies throughout the study period (Committee on the Robert Wood Johnson Foundation Initiative on the Future of Nursing, 2011). To support the Committee's work, various research papers were commissioned to offer expert views from a network of professionals and experts. The Committee's goal was to identify the vital role of nurses in designing and implementing more effective and efficient health care systems. The report identified four key messages that formed the basis of the Committee's discussions. The messages are discussed in detail below and highlight how each message has transformed professional nursing practice since the 2011 report publication
Key Messages Transforming Nursing
The report notes that some states have specific provisions on the limitation of nurses' authority and practice. Other states have vague requirements challenging to break down (Committee on the Robert Wood Johnson Foundation Initiative on the Future of Nursing, 2011). Many underutilized practitioners are suffering under the healthcare system by denying advanced practice registered nurses (APRNs) the authority to see patients or prescribe medication. The key message has influenced various state regulations, as since the report, nine states, including Connecticut, Maryland, and Minnesota, have allowed nursing practitioners full practice and prescriptive authority (Committee for Assessing Progress on Implementing the Recommendations of the Institute of Medicine Report, 2016; Threw, 2019).
The report noted that the nature of the health sector was ever-evolving, and as a result, nurses need to acquire higher levels of education to keep at par with the industry. Such education should be conducted both as a prerequisite to getting their licenses and after as well. The Committee recommended that nurses enter the workforce with a baccalaureate degree. Further, a more significant number of nurses should consider advanced degrees. At the time of the report, approximately half of nursing practitioners held baccalaureate degrees (Committee for Assessing Progress on Implementing the Recommendations of the Institute of Medicine Report, 2016). The number has increased to 58% in 2018 (Campaign for Action, 2020), with the 80% target projected to be reached by 2029 (Threw, 2019).
Leaders in nursing practice should be proactive and work together with other physicians to redesign the country's healthcare system. These efforts require strong leaders. Leaders should work together with other health practitioners to identify problems and develop improvement plans. Nurses must be involved in health policymaking decisions by playing an active role in boards and commissions where policies are deliberated. As of January 28th, 2020, the Campaign for Action notes that 7,100 nurses serve on different boards (Campaign for Action, 2020).
Accessing health care workforce data is essential to improve the infrastructure while promoting policy and business development. Current data informs poli...
