Write an informal presentation (500-700 words) to educate nurses about how the practice of nursing is expected to grow and change. Include the concepts of continuity or continuum of care, accountable care organizations (ACO), medical homes, and nurse-managed health clinics.
Share your presentation with nurse colleagues on your unit or department and ask them to offer their impressions of the anticipated changes to health care delivery and the new role of nurses in hospital settings, communities, clinics, and medical homes.
In 800-1,000 words summarize the feedback shared by three nurse colleagues and discuss whether their impressions are consistent with what you have researched about health reform.

The health care delivery system in the US is undergoing critical changes. Nurses will experience a larger share of the impact. The process will cause an increase in the number of nursing jobs available, especially in the community. The category will have fewer jobs in acute care medical facilities. Nurses will experience growth and observe changes in the sectors of the continuity/continuum of care, medical homes, Accountable Care Organizations (ACO), and nurse-managed health clinics. Changes in health care delivery structures will create new roles and expectations for nurses in medical facilities, medical homes, hospitals, and communities. The essay elaborates on the changes and the anticipated the outcome of the reforms on nursing practice in the provision of health care services.
The first aspect deals with the continuity of care. The medical world defines the concept as the extent to which a few nurses have the assignment to care for particular patients (Siow, Wypij, Berry, Hickey & Curley, 2013). A concept is an approach that focusses on a patient, such that the care services a nurse provides has a basis for the needs of the patient and the family. Such a plan has more benefit to a patient with a high level of acuity because such patients have more needs. Medical practitioners believe that health care providers can effectively meet the needs through a proper synergy of efforts between patients and nurses. Provision of care based on the individual needs of a patient would optimize the expected results of the treatment, and the relationship between nurses and patients. Patients have indicated that there is an importance in a patient knowing the nurse concerned, just as the nurse has of the patient (Siow, Wypij, Berry, Hickey & Curley, 2013). The existence of such a relationship enables a nurse to gather more information that will be useful when making decisions on care practices that would be most suitable to the patient and the family involved. The patient will be an active participant in the treatment process. The involvement will have a positive influence on the quality of care and reduce the risk of undesired occurrences. Implementation of the plan will result in better collection and understanding of relational, informational and managerial aspects of the continuum of care (Siow, Wypij, Berry, Hickey & Curley, 2013). One can make a better observation of the impact in outpatient setups. The step will cause a general improvement in the quality of healthcare for patients who require intensive care.
The second aspect deals with Accountable Care Organizations (ACO). An ACO is a program developed by the joint efforts of health professionals, inclusive of basic care doctors and a medical facility, with the aim of taking responsibility for the cost and quality of healthcare for the patients who use the services the organization provides ("New Care Delivery Models in Health System Reform: Opportunities for Nurses & their Patients", 2018). The team serves a specific population of patients and encourages the redesign of healthcare processes and investment in infrastructural aspects to facilitate excellent quality of treatment and efficient systems of delivery. Earlier versions of the setup had a basis in a clinic or doctor. Current modifications have based the practice around ACO professionals, who are people capable of providing organization and leadership within the structure. Such could be a practitioner or a physician. The scope of practice that nurses follow is compliant with the use of new models of delivery such as the Accountable Care Organizations.
