Alzheimer’s disease is a condition with increasing incidence in our population. There is no differential diagnosis, so the onset of the disease may go unnoticed by others. By the time the patient reaches the moderate stage of the disease, manifestations are quite apparent to others and the required level of care often places tremendous strain on the caregiver. Caregiver role strain is a nursing diagnosis that is frequently applied in the situation.
After reading the above information, discuss the following points (minimum of 250 words):
• Since patient wandering and confusion are common for the patient with moderate-to-severe Alzheimer’s disease, what would the RN teach the family about maintaining a safe environment? Provide three examples.

Wandering and confusion that characterizes Alzheimer’s disease can lead to accidents. These accidents can lead to trauma and even fractures. It is because of such reasons that it is necessary for the family of the patient to maintain a safe environment through keeping walkways well-lit by adding extra lights on stairways, doorways, and areas between rooms (Ballard et al., 2011). The family could also remove tripping hazards by removing objects such as floor lamps, coffee tables, and magazine racks. Lastly, disable and remove guns or other weapons. Their presence may lead to unexpected danger since a person with Alzheimer’s may mistakenly take a familiar caregiver to be an intruder.
Effective care by the family is dependent on the information provided by the nurse. The nurse should ensure that the family understands the information provided to them fully by adapting her teaching to the family’s educational level, culture and socioeconomic status. The nurse can achieve this by taking into consideration the cultural values of the family so that the information provided does not conflict with those values. Secondly, the use of simple language rather than medical jargons will enhance quick understanding by the family (Swallow et al., 2009).
