Strategic Delivery of Healthcare
Continue to build upon the strategic plan. Update your strategic plan based on your instructor’s feedback from your Module 1 SLP submission. Once the update is completed, add the following sections to your strategic plan in 2–3 pages:
Strategic Plan Timeline: In this section (1 paragraph), identify a realistic timeline for the accomplishment of your plan. You should state the expected start and end dates.
Short-Term Goals: Identify 2 or 3 measurable short-term goals for your strategic plan and the anticipated date/duration to meet these goals. These can be provided in bullet point form.
Long-Term Goals: Identify 2 or 3 measurable long-term goals for your strategic plan and the anticipated date/duration to meet these goals. These can be provided in bullet point form.
Operational Plan: In this section, further explain your short- and long-term goals. Explain your “road map” to completion of these goals, and match an “activity” to each “goal.” Essentially, the activity is the action taken to reach the goal.
To accompany your explanation, use a Gantt Chart for a visual display (see image below). For more information on how to create a Gantt Chart in Excel, review the following sources:

References: Include all reference material used to develop these sections. (The Reference section is not to be included in the page count).
Note: At this point, your plan should be at least 4–6 pages, not including the title and reference pages. You should have 5–7 references at this point.

Timeline
The focus is on the adoption of electronic health records in regions such as West Virginia, which the CDC (2022) reveal has one of the highest numbers of people diagnosed with diabetes. This implies that the mission statement will be “to improve patient safety and quality care of outpatient diabetes patients,” while the vision statement will be “to ensure West Virginia is diabetes free in the long run.” As such, there is a need to introduce a timeline that would facilitate the realization of the above objectives. The start date for this plan will be Apr. 10, 2023, up to Aug. 10, 2023. The focus is on approximately four months to accomplish the plan, especially the short-term goals. The plan runs for months because it involves the acquisition of medical and technological equipment and the need to train stakeholders, including patients and healthcare practitioners, regarding the benefits of electronic health records and how to apply them to achieve positive outcomes.
Short-Term Goals
To improve patient safety and quality care of outpatient diabetes patients (1st Short-term Goal)
Since the number of diabetes patients in West Virginia is high, there is a need to manage it efficiently to reduce the adverse impacts on one’s health, including the prevalence of chronic conditions such as cardiovascular complications and kidney, or worse, death. There is a need to improve patient safety and quality care, which revolves around minimizing instances of hospitalization and the costs one incurs in the process. This primary short-term goal will take about four months to achieve.
To reduce hospital visits (2nd Short-term goal)
Since the focus on EHR includes improving the organization and exchange of patient data, one can conclude that they can receive medical advice without necessarily adopting the in-patient hospital approach, thus increasing flexibility. This is also a cost-effective technique. This goal should take approximately two weeks to 2 months to achieve.
To manage diabetes symptoms efficiently (3rd Short-term goal)
In this regard, Veinot et al. (2010) allude that EHR is adopted to enhance clinical decision-making, improve communication among healthcare stakeholders and manage large amounts of data. This implies that healthcare practitioners have access to data that can facilitate research and containment of diabetes symptoms. In other words, since it documents diagnosis and interventions, the patient’s access to information on the management of symptoms is expected to be efficient. The anticipated duration for this goal is between 1 and 3 months.
Long-Term Goals
Long-term goals can be tied to the vision statement highlighted above.
To ensure West Virginia is a diabetes-free state in 10 years time (1st Long-term goal)
The goal for the future is to ensure that the state reduces the cases of diabetes in a decade’s time. This includes the adoption of EHR systems that facilitate documentation of diagnostics and treatment that can be shared among healthcare practitioners to improve healthcare outcomes among residents of West Virginia.
To reduce government spending on diabetes management (2nd Long-term goal)
According to the American Diabetes Association (2021), West Virginia spends approximately ...
