The term comprises the selection of a specialist in one particular patient in a healthcare system, who knows the aliment of the individual and carries out specific and extensive coordination of other practitioners on what the patient needs. Individuals who are tasked with the responsibility of providing primary healthcare to patients are called Primary Care Physicians (PCPs) (Heidelbaugh, 2018). They are on the front lines of population health management and preventive care where they ensure that their work is effective by gaining trust from their assigned patients in order to deliver personalized care.
