Throughout this course you will be developing a formal, evidence-based practice proposal.
The proposal is the plan for an evidence-based practice project designed to address a problem, issue, or concern in the professional work setting. Although several types of evidence can be used to support a proposed solution, a sufficient and compelling base of support from valid research studies is required as the major component of that evidence. Proposals are submitted in a format suitable for obtaining formal approval in the work setting. Proposals will vary in length depending upon the problem or issue addressed; they can be between 3,500 and 5,000 words. The cover sheet, abstract, references page, and appendices are not included in the word limit.
Section headings and letters for each section component are required. Responses are addressed in narrative form in relation to that number. Evaluation of the proposal in all sections is based upon the extent to which the depth of content reflects graduate-level critical-thinking skills.
This project contains seven formal sections:

Abstract
In any healthcare facility around the world, one of the most common issues that occur are CAUTIs. CAUTIs stand for Catheter Associated Urinary Tract Infection, which is an infection caused by bacteria which lives in in-dwelling catheters. As most health-care facilities and workers know, the importance of finding ways to resolve this issue, lies in the fact that the mortality rate for CAUTIs are estimated to be about 10% of hospitalized patients who are using in-dwelling catheters. Aside from this, CAUTIs costs a lot, not only for the patients but also for the administration of the healthcare facility. Considering that CAUTIs are supposedly preventable, this led many fields and organizations to focus in preventing this. Some organizations include WHO and CDC which regularly establishes guidelines in order to reduce cases of CAUTI. However, studies states that CAUTI cases are still very prevalent today. Perhaps, this is due to the fact that many nurses are not aware of the guidelines that have been set by these organizations. And this is where the role of the Nurse-manger becomes critical. Thus, in this paper, the relationship between effective nurse management as well as number of CAUTI cases would tackled.

Section A
Organizational Culture and Readiness Assessment
Implementation of evidence base practice (EBP) has been highly considered due to the high scrutiny and tighter regulations of the patients' outcome. Implementation of EBP brings along a change in the organization's culture since new behaviors and practices must be adopted. This is a proposal for the implementation of EBP in the organization. It evaluates the culture of the Organization and its readiness to implement EBP.
EBP plays a central role in the mission and philosophy of this organization. However, the degree to which it is being practiced is not enough. There is still a void to be filled. This is because, as much as the administration is committed to the implementation of EBP regarding planning and support, the nurses and physicians are not committed as much.
According to Melnyk & Fineout-Overholt, (2014), the process of EBP focuses on incorporating good information-seeking habits into a daily routine. Nevertheless, lack of commitment by the nursing staff and physicians is as a result of lack of knowledge and skills of using EBP. The number of the nurse scientists who could assist in research if need be is inadequate. To be precise, there is only one! The advanced practical nurses that can mentor the staff on EBP are also inadequate. This makes it difficult for the practitioners to model EBP in their practice (Marita, 2008).
However, the organization is equipped with quality computers which make it easy for the electronic access to research. The bigger number of the have got proficient computer skills. The librarians too have a little knowledge about EBP. They are not so much involved in the research, but they are willing to. The fiscal resources have not yet been utilized fully in EBP, but the organization's team is willing to. Sharing information has been part of the organization's culture. Decisions are mostly made by the administration, but the opinions of the rest of the staff are put into consideration. In overall, the organization is ready, but it is not acting.
Nurses will embark on EBP by starting with clinical inquiry (Susan, 2010). This is the ongoing process of questioning and evaluation which is later applied to practice. To integrate clinical inquiry, the organization will engage multidisciplinary administrative that will ensure that nurses gain the required knowledge (Laslie, 2015). They will also ensure that the nurses are motivated to take part in EBP dissemination, initiatives to improve quality and research.

Problem Description
In maintaining quality healthcare, nurses serve as the forefront which acts as “front lines” in monitoring, evaluating, and also delivering the standard and up-to-date knowledge that would not only assure standard healthcare procedures but even improve it. In other words, nurses do not only function as those who deliver the technologies, methods, and strategies for patient care that they’ve learnt through technical knowledge, but also acts as those who could evaluate which knowledge is useful, and what changes could be made in order to further better health services (Stevens, 2013). As according to McCann (2014), mistakes in a patient's medical among other “preventable medical errors” presents as one of the greatest causes of mortality among patients in the United States, ranking third just after cancer and heart disease. Among these ‘preventable medical errors’ include mistakes which cause problems such as catheter associated urinary tract infection (CAUTIs) or Catheter Related Urinary Tract Infections.
