PICOT Question and PICOT Problem on Nursing Shortage

Essay Instructions:

Assessment Description

Review your problem or issue and the study materials to formulate a PICOT (Patient, Intervention, Comparison, Outcome and Time) question for your capstone project change proposal. A PICOT question starts with a designated patient population in a particular clinical area and identifies clinical problems or issues that arise from clinical care. The intervention used to address the problem must be a nursing practice intervention. Include a comparison of the nursing intervention to a patient population not currently receiving the nursing intervention, and specify the timeframe needed to implement the change process. Formulate a PICOT question using the PICOT format (provided in the assigned readings) that addresses the clinical nursing problem.

The PICOT question will provide a framework for your capstone project change proposal.

In a paper of 500-750 words, clearly identify the clinical problem and how it can result in a positive patient outcome.

Step 1: Create PICOT question; A PICOT question is presented and provides a clear framework for the capstone project change proposal. Your PICOT question should clearly outline all of these elements: patient, intervention, comparison, outcome and time.

Step 2: PICOT Problem: Identify the PICOT problem, what clinical problems or issues may arise from clinical care? The PICOT problem as it relates to evidence-based solution, nursing intervention, patent care, health care agency, and nursing practice is thoroughly described

Step 3: Describe nursing intervention: A nursing intervention used to address the problem. Compare the nursing intervention to a patient population not currently receiving the nursing intervention, and timeframe needed to implement the change process.

Step 4: Summarize Clinical Problem and Patient Outcome: The clinical problem and how it can result in a positive patient outcome.

PICOT Question
The nursing shortage is a concerning issue in the modern American healthcare system, threatening to trigger diverse adverse outcomes. Foli et al. (2021) note that healthcare departments with a nursing shortage often increase the probability of more prolonged admissions, compromised care, medical errors, and diminished patient satisfaction. Designing a PICOT question can assist the stakeholders in determining the value of increasing nursing staffing and improving outcomes. In this context, the population represents the project’s target patients at risk of compromised health due to the unavailability of nurses. Inpatients in hemodialysis centers require close monitoring by these healthcare providers, making adequate staffing fundamental. The intervention in this context is increasing the number of nurses through diverse evidence-based approaches. Comparison with patients in a similar setting but one that grapples with nursing shortage will be fundamental to illustrate the impacts of the intervention on outcomes. Finally, this project will determine the outcome’s impact by analyzing factors such as length of hospitalization and medical errors after 14 days of implementation. The PICOT question will be, in inpatients in hemodialysis centers, does an increased number of nurses, compared to nursing shortages in the units, lead to improved health outcomes within 14 days?
P - inpatients in hemodialysis centers
I - increased number of nurses
C – nursing shortages
O – improved health outcomes
T – 14 days of hospitalization
Identify the PICOT Problem
Nursing shortage or understaffing has become problematic due to the diverse consequences associated with the issue in healthcare settings. Bakhamis et al. (2019) indicate that nurses become overburdened and experience poor job satisfaction and burnout, leading to compromised patient safety. This issue triggers complex clinical problems like medical errors, delayed care, hospital-related infections, and bedside falls, among other adverse outcomes. These concerns reflect Jarrar et al.’s (2018) observations about the value of introducing evidence-based interventions to combat the understaffing of nurses and revert the adverse clinical consequences. The researchers acknowledge that this intervention requires concerted efforts from different healthcare agencies focusing on patient-centered care to improve the nursing practice and transform care.
