DEVELOPING YOUR PROJECT PROPOSAL
The Session Long Project of this course will be where you begin the process of becoming an expert on the problem you have chosen to address by assembling the scholarly materials gathered for your Case assignments into an annotated bibliography (annotated bibliographies were introduced in MHS 504). Your bibliography should conform to American Psychological Association (APA) formatting guidelines. You will accumulate sources and write annotations as you gather new scholarly materials every week and submit updated versions of your bibliography at the conclusion of each module (the final version of your annotated bibliography is due at the conclusion of Module 4). Remember, your annotations should capture sufficiently the essence of what the source document was about and any critical details you will want to be able to recall as you begin drafting your report, plan, etc.; for this reason, tremendous care should be taken in drafting your annotations.
Your Task for This Assignment
For Module 1, identify a minimum of 5-8 scholarly sources (at least 5 of which must be peer reviewed) and provide annotations for each. Upload this initial draft of your annotated bibliography to the SLP 1 Dropbox by the conclusion of this module.

You are expected to consult the scholarly literature in preparing your paper; you are also expected to incorporate relevant background readings.
Your paper should be written in your own words. This will enable me to assess your level of understanding.
Proofread your paper to ensure that grammar and punctuation are correct, that reference citations conform to APA guidelines, and that each part of the assignment has been addressed clearly and completely.
Your assignment will not be graded until you have submitted an Originality Report with a Similarity Index (SI) score <20% (excluding direct quotes, quoted assignment instructions, and references). Papers not meeting this requirement by the end of the session will receive a score of 0 (grade of F). Do keep in mind that papers with a lower SI score may be returned for revisions. For example, if one paragraph accounting for only 10% of a paper is cut and pasted, the paper could be returned for revision, despite the low SI score. Please use the report and your SI score as a guide to improve the originality of your work.

In this article Herman and Cefalu argue that the enormous diabetes disease not only affecting individuals within the United States but also around the world. They state that the control of this chronic disease demands for effective approaches like patient education and team care for the patients. They identified that in 2012 in United States $245 billion were used for the diabetes programs. Considering the poverty level of the United States population the diabetes disease is likely to have a negative effect to the economy. From their suggested approaches it is possible to improve the level of care which can be provided for the society especially those who are suffering from diabetes. Their research is a clear depiction of the situation of diabetes within the society.
Patient-assessed health outcome measures for diabetes: a structured review, CITATION Gar02 \l 1033 (Garatt, Schmidt, & Fitzpatrick, 2002)
Geratt, Schmidt and Fitzpatrick’s main objective for this article was to identify disease specific measures of health-related quality of life majorly for the diabetes and identify the reliability, validity and responsiveness of the instrument used within the diabetes programs. They used systematic research techniques to identify the instruments. In their research they engaged extensively other authors whom might have substantial knowledge pertaining the diabetes care. From their research they generated 252 references for the instruments which can be used for diabetes curative measures. Even though for the instruments that they stated none of them has been tested for its efficiency within the health care they believe that they can impact positively when implemented. In their conclusion they stated some of the best evidence based approaches which can be used for the diabetes situation for the internal and external validity to the society. In their article they stated ADDQoL, DPH-1/18, DSQOLS, D-39 and QSD-R as some of the best instruments which should be used. From this approach it would be easier for the health sector to implement and advance the new interventions for chronic diabetes to greater levels.
