I gain strength from God. I also gain strength from my 4 children. I know what it means to them to have me in their life.

I feel a bit confused right now as to my philosophy of life. I believe things happen for a reason, whether or not we understand what that reasoning is. I believe in prayer but sometimes question whether our life is pre-determined,

4-What type of spiritual/religious support do you desire?

I desire inner peace and fortitude. I feel good when I attend church services even though I have fallen out of that practice for at least a few years. Being the turmoil my life has become recently, I have been craving visiting my church, even if it's just to be there by myself. It feels very peaceful when I'm there. It gives me strength to go on.

5-What is the name of your clergy, ministers, chaplains, pastor, rabbi?

I have attended Epiphany catholic church for many years. The monsignor is my favorite, Monsignor O'Daughtery. Very kind and inspiring man.

6-What does suffering mean to you?

Suffering means inner turmoil to me. Not being able to help those I love. Seeing people not be able to control what happens around t--hem, being in pain or not being able to provide our most basic needs.

7-What does dying mean to you?

I have no doubt our souls continue on. Our bodies are just a temporary vessel. I don't necessarily believe in "Hell". I believe there is an incredible experience waiting for all of us. Why would God make us suffer?

8-What are your spiritual goals?

My spiritual goals are to gain strength so I may continue to help those around me. I am very sensitive to my surroundings and people's feelings. I feel so overwhelmed sometimes when I can't change what is happening. I need strength and inner peace.

I was brought up in the Catholic faith attending church services regularly. I continued that role with my children. In recent years I have fallen out of that routine. I now find myself wanting to return.

Many days I don't even want to get out of bed. I think of my children and how I MUST push forward. For them and for me. I ask God and my mothers to help me. I ask for a lot of guidance. I need it. Your spiritual needs assessment survey must include a minimum of five questions that can be answered during the interview. During the interview, document the interviewee's responses.

The transcript should include the questions asked and the answers provided. Be sure to record the responses during the interview by taking detailed notes. Omit specific names and other personal information through which the interviewee can be determined.

Part II: Analysis

Write a 500-750 word analysis of your interview experience. Be sure to exclude specific names and other personal information from the interview. Instead, provide demographics such as sex, age, ethnicity, and religion. Include the following in your response:

Introduction
Spiritual assesment is an important practice that helps AIDS health workers to identify areas where a patient and or hi/her family may need spiritual support. Spiritual support is part of providing holistic care and has far reaching psychological effects on th patient that renew their strength against the disease. The application of the skills learnt on this imperative form of palliative care led me to conduct an interview on a 55-year old Hispanic, Catholic woman. She is married and has 4 children whom she has raised as Christians. She is also working and she has been struglling with her condition for a few years now. In the afore-included interview, she seemed to have accepted her condition but she is hopeful that God can deliver her from her suffering and she seemed to have amplified her spiritual concerns.
What Went Well
During the interview, several things went well. The first one was the environment in which the interview was held. The environment is an important element in conducting such interviews as spiritual issues are personal and most people prefer a tranquil and serene surrounding which implicates privacy and allows the client to voice their opinions freely.
