Both diseases are related since diabetes could damage the small blood cells, leading them to develop hypertension (CanopyHealth, 2018). Therefore, it is essential to involve an interprofessional team to ensure that the patients receive the adequate care they need since a combination of these diseases could lead to adverse effects such as stroke or hypertension. A good interprofessional team will ensure a proper collaboration so that the patient receives the healthcare they need anywhere and at any time.
Presenting best practices is essential to ensure an interprofessional team manages diabetes and hypertension simultaneously. One of these practices is adopting technological tools that perform functions such as monitoring the patient and measuring their insulin levels (Muegge & Tobin, 2016). When patients wear these gadgets, healthcare providers can monitor their progress remotely. The data collected can also be sent to all the members of the interprofessional group to administer the help the patient needs at whatever time. Therefore, adopting technological tools is one of the best practices for simultaneously managing a patient with diabetes and hypertension.
