However, most healthcare organizations recognize that it can be challenging when trying to implement such a plan. Transitioning from service- to value-based system requires careful analysis and understanding of key components that would increase chances of successful implementation. The paper explores the factors to consider when implementing pay-for-value in regards to considering needs of providers, cost, and culture. It also examines how communication, measurement, and physician satisfaction can influence efforts at population health management.
One factor to consider when implementing a pay-for-value plan is needs of providers and care teams. Gruessner (2016) claims that providers workloads are already overwhelming. When implementing pay-for-value system, it is crucial to consider support for providers because they carry the most responsibility during the transition (Gruessner, 2016). It is vital for a health administrator to plan how support for providers so that they can accomplish their responsibilities effectively. For example, ensuring that resources and tools such as electronic health records and efficient medical equipment are available in the healthcare facility to make work easier. Providers and care teams cannot provide quality care unless they have resources that facilitate their work. Implementing a pay-for-value plan should be designed in consideration that providers also feel overwhelmed. Aspects such as flexible scheduling, stress management support, and financial incentives can ensure such support. The more support providers get, the better they can provide quality care.
Cost is another important consideration when implementing pay-for-value plan. When transitioning to a value-based plan, additional costs can be incurred. For example, additional information technology to ensure care coordination between different care teams (Ryan, 2018). Additionally, care management staff such as specialists may need to be recruited to ensure patients can attend their appointments without being delayed. If the healthcare organization works with people with diabetes, specialists such as dietician, eye doctor, or endocrinologist should be available to serve patients in real-time. Patients should not wait for referrals for days or weeks before attending their appointments. Further, providers need to be reimbursed for their efforts. Performance incentives are part of pay-for-value system and so administrators should consider such costs. In this way, implementing a pay-for-value plan means additional costs that the healthcare organization did not encounter previously (Ryan, 2018). Planning for cost should be prioritized to avoid components of the plan failing.
An administrator should also consider culture when implementing a pay-for-value plan. Ratna (2019) argues that when developing a health plan, the beliefs, customs, and needs of the community being served should not be ignored. The uniqueness of a community in regards to customs, politics, economics, and demographics should be examined closely. For example, if the healthcare organization serves a large precent of Hispanic population their cultural needs should be incorporated in the plan. It is possible that many do not speak English or have little understanding of the language. The way they approach care may also differ from mainstream provision of care.
