An open culture implies failure and public trust breach. The fear of being punished makes a medical professional increasingly reluctant to reveal mistakes. Revealing the medical error could also generate shame and blame from other healthcare providers and disciplinary action from their professional organizations. The open culture could communicate that the physician is to blame for the error, resulting in job loss. Therefore, revealing the medical error could also tarnish the organization’s or healthcare professional’s reputation, particularly due to the undesirable connotation of the term “mistake” (Rodziewicz, Houseman, & Hipskind, 2021).
