Health Initiative The population selected for the health initiative is the state of Massachusetts. The health need identified for this population is the high prevalence of chronic diseases, including heart disease, cancer, diabetes, and obesity. According to (Bor et al. , 2023) these chronic diseases are responsible for most deaths and healthcare costs in the state. Therefore, it is necessary to implement a health program that aims to prevent and manage chronic diseases effectively. The planning model that aligns with the health initiative is the PRECEDE-PROCEED model. This model is a comprehensive planning framework that includes several phases, from assessing the population's health needs to evaluating the program's outcomes (Chowdhury et al., 2016). The PRECEDE phase includes evaluating the social, epidemiological, behavioral, and environmental factors contributing to health needs. The PROCEED phase includes planning, implementing, and evaluating the health program. The PRECEDE-PROCEED model is suitable for the health initiative in Massachusetts as it considers the complex nature of chronic diseases and the social determinants of health (Horberg et al., 2023).
