The United States of America has a population of more than 333 million people, which is expected to be roughly 373 million in 2053. The nation is backed by one of the most intricate healthcare systems in the world, as it consists of interlinking relationships among healthcare providers, payers, and patients receiving care. The country’s healthcare system constantly evolves as time goes by. Public and private healthcare sectors acquire payment from private and public financial bodies. The hospitals mainly receive payment via a diagnostic-connected group (DRG) that assigns setoff payment for a given issue or treatment sequence. To ensure that Americans enjoy prime health as individuals or as a population, they need to have the remuneration of optimal healthcare services that are effectively and efficiently synchronized within a solid public health system.
The U.S. health system does not have an identical system and only is exercised under gradually adjusting legislation. Over the past few decades, different laws have been introduced by the federal government to try to find solutions to several problems affecting the system's effectiveness. One of the healthcare reform laws that has been enacted and transformed the U.S. healthcare system was the 2010’s Patient Protection and Affordable Care (the Obamacare or Affordable Care Act). The legislation asserts that healthcare insurance organizations should deliver a given level of coverage with every single plan (Davalon, 2022). Obamacare covered several coverages, including emergency care, hospitalization, pediatric care, preventative care, laboratory services, and mental health care.
Over the last five years, the country has experienced the introduction of other healthcare regulations. The H.R. 728: Title VII Nursing Workforce Reauthorization Act of 2019 is a popular care legislation in the United States. The bill aimed to enhance public nursing education, retention, and practice. The law proposes the development of the grant to build career ladder programs responsible for assisting the healthcare employees like Certified Nursing Assistants (CNAs) to become baccalaureate-ready registered nurses (RNs) or nurses with graduate nursing education (McMullen & Travers, 2023). Title VIII proposes that the programs should be initiated for vulnerable communities ensuring they also receive financial aid.
The administration of former President Donald Trump also introduced various changes to the Medicaid and Medicare programs. The 2019 declaration of the Primary Care First model aimed at simplifying the payment of primary care doctors (Tikkanen et al., 2020). As of 2020, other changes were enacted in the Affordable Care Act. The administration of Trump halted various consumer protections via executive and regulatory actions. The Trump administration removed personal mandates and financial penalties for failing to have health insurance (Tikkanen et al., 2020). Furthermore, the management declared its objective of addressing high healthcare prices, particularly prescription medications. Two bills were passed in 2018 that expelled gag clauses (Tikkanen et al., 2020). The two clauses prevented pharmacists from informing the clients whenever the drug price was less than the insurance-discussed price. These changes in the structure have been critical in changing how the U.S. healthcare system functions.
The federal government introduced various actions to reform and restructure and responsibilities of the nurses in the healthcare system. The new system requires nurses to identify and guide individuals seeking to pursue an occupation in nursing. They should partner with high school counselors to guide nursing scholars. Because there are vast training and education opportunities, nurses must embrace the change and provide quality healthcare. Because of spousal poverty, patients can be restricted from accessing proper medical care. Therefore, the new system introduced a rule that nurses should recognize such patients and notify them of the changes to help them acquire comprehensive treatments.
The actions incorporated would automatically have significant impacts on the nurse’s roles and responsibilities. For example, if the bills are passed, the R.N.s will have access to grants for clinical nurse specialist education. With such a level of skills and knowledge, nurses can offer a series of care to society. Therefore, nurses will effectively apply research and coordinated care to promote patient outcomes. Moreover, the bill will enhance residency and internship initiatives to facilitate RNs' transition to the development of specialties among nursing scholars.
