Introduction
The term approach to care could be elucidated as the type of care provided to patients because that particular care is suitable for that particular patient (Matzo and Sherman, 2010). It follows that using effective approach to care enables patients to heal better because they feel good about themselves in addition to enabling family members of the patient to communicate effectively. Mr. P, a 76-year-old male with congestive heart failure has been hospitalized frequently. The patient has difficulty managing his polypharmacy and maintaining diet restrictions. Mr. P also has moist crackles in the lung fields, pitting edema, and labored breathing. The patient only has a wife as a family member who complains about the medical bills and her inability to leave the house. Ultimately, Mr. P asks why God allows him to continue living. The upcoming paragraphs describe an approach to care, a method for educating family members, recommend a treatment plan, and provides an appropriate teaching plan for Mr. P and his wife.
Approach to Care
According to Azad and Lemay (2014), a palliative care approach is suitable for congestive heart failure patients. In fact, the same author highlights that it is relevant for elderly patients with advanced cases of congestive heart failure. It is notable that Mr. P is exhibiting symptoms such as moist crackles in the lung fields, pitting edema, and labored breathing. These are symptoms for advanced cases of congestive heart failure implying that a palliative care is appropriate for the patient. It follows that a six-month prognosis needs to be determined before enrolling Mr. P in a hospice program. Further, Mr. P must consent to on-aggressive care, which is part of a palliative care. Mr. P may also be initiated into a hospice program, which will be particularly useful if his life needs to be terminated abruptly.
