Family Health Assessment Part I
Understanding family structure and style is essential to patient and family care. Conducting a family interview and needs assessment gathers information to identify strengths, as well as potential barriers to health. This information ultimately helps develop family-centered strategies for support and guidance.
This family health assessment is a two-part assignment. The information you gather in this initial assignment will be utilized for the second assignment in Topic 3.
Develop an interview questionnaire to be used in a family-focused functional assessment. The questionnaire must include three open-ended, family-focused questions to assess functional health patterns for each of the following:

Nutrition
Sleep/Rest
Elimination
Activity/Exercise
Cognitive
Sensory-Perception
Self-Perception
Role Relationship
Sexuality
Coping
Select a family, other than your own, and seek permission from the family to conduct an interview. Utilize the interview questions complied in your interview questionnaire to conduct a family-focused functional assessment. Document the responses as you conduct the interview.
Upon completion of the interview, write a 750-1,000-word paper. Analyze your assessment findings. Submit your questionnaire as an appendix with your assignment.
Include the following in your paper:
Describe the family structure. Include individuals and any relevant attributes defining the family composition, race/ethnicity, social class, spirituality, and environment.
Summarize the overall health behaviors of the family. Describe the current health of the family.
Based on your findings, describe at least two of the functional health pattern strengths noted in the findings. Discuss three areas in which health problems or barriers to health were identified.
Describe how family systems theory can be applied to solicit changes in family members that, in turn, initiate positive changes to the overall family functions over time.
Cite at least three peer-reviewed or scholarly sources to complete this assignment. Sources should be published within the last 5 years and appropriate for the assignment criteria.
Prepare this assignment according to the guidelines found in the APA Style Guide, located in the Student Success Center. An abstract is not required.

Introduction
Family health assessment is a critical technique of examining a family, and it can help in determining where the members of the family should focus on improving the overall health of the family. The functional health patterns offer eleven functions which are vital in assessing families and individual members. The literature uses Gordon’s functional health patterns to assess the health of a family of four members; the members will be asked open-ended questions based on the eleven categories.
Family Health Assessment
Johnson’s family was chosen to participate in the assessment. The Johnson’s are family friends who were willing and also gave their consent. This was critical since some of the questions asked during the interview were invasive since the family would be required to discuss private issues and address health matters that the family members have encountered. The family comprised of the husband, wife, and their two daughters one aged fifteen years and the other eight years. This is an African-American family from a low social class, and they are all Christians. Both the father and the mother have to work hard to ensure that they support their family.
Assessment findings
Health perceptions
The questions asked in his case helped in understanding the impression and notion of the family members about their health. The wife indicated that she has been diagnosed with diabetes but maintained that she keeps in touch with her physician, who monitors her condition. The husband too has issues with weight and is taking medicines for high blood pressure. However, the two girls and healthy, and they have no health complications. In order to maintain their health, the family undergoes health checkups every year and state that their goal is to improve their health and protect their daughters from foreseeable health complications.
Nutrition
The family maintained that they like taking food rich in fats but emphasized that they also take fruits and vegetables. The husband also indicated that he like taking his family out, so they also eat from restaurants regularly. Also, every member takes at least three meals a day; the girls also eat snacks. The family consumes about 1500-2000 calories in a day. The family has access to balanced food, particularly meals rich in proteins, carbohydrates, vitamins, and fats, among other components.
Sleep/Rest
The family indicated that every member obtains about eight to nine hours of sleep at night. The wife stipulates that everyone has to sleep early around 9 AM so that they can have enough time to rest and wake up before waking about 5 to 6 AM the next day. They, however, indicated that during weekends, they are likely to sleep late but not past 10 PM. The responded that there are no factors preventing them from assessing enough sleep. All members of the family indicated that they have regular sleep patterns.
Elimination
Mrs. Johnson reported that she and the older daughter had had constipation issues but maintained that they had increased their water and fiber intake. The young daughter sometimes uses the restroom very late at night. All members indicated that urinate more during the day than at night and that there are no health problems affecting their bowel and bladder.
Activity/Exercise
The family members participate in different activities and exercises. Mr. Johnson loves playing basketball, participates in camping with his family. His wife has to engage in physical activities because of her diabetic condition; she, therefore, participates in walking and going for cross country. The daughters are dancers and also sometimes accompany their mum when she takes part in running. They all agreed that they like taking part in physical activities, and there are no factors that prevent them from doing so.
Cognitive
There are numerous cognitive defects that may be prevalent in individuals; these include initiating speech, recalling information, and paying attention (Sinai, Hassiotis, Rantell & Strydom, 2016). Johnson’s family indicated that they have a problem of remembering. Mrs. Johnson noted that she normally forgets where she keeps her phone. The girls sometimes forget working on their homework. The members stated that they are improving their remembering problem by improving their memory by ensuring that they get enough sleep. No member has problems learning new things.
Sensory-perception and Self-Perception
All family members are comfortable with their sensory perception, and there were no factors to suggest sensory aid. Also, there were no indications of sensory deprivation. Mrs. Johnson emphasized she is able to point out the slightest tastes that nobody can. Everyone in the family was happy about their family; Mrs. Johnson remains positive despite having diabetes. The family is worried about their financial situations since the parents work hard to support the family; this is a concern since in case of medical emergencies, it would be impossible to meet the medical expenses. The goal of the family is to improve their income and promote their health.
Role relationship
Although Mr. Johnson is the head of the family, he allows other members to make decisions regarding their eating, health issues, and house rules. Regarding the roles, Mr. Johnson participates in cooking and deals with discipline. On the other side, Mrs. Johnson does laundry and other cleanliness matters; although the girls have nor major responsibilities, they help their parents.
