Evolving Practice of Nursing and Patient Care Delivery Models
The RN to BSN program at Grand Canyon University meets the requirements for clinical competencies as defined by CCNE and AACN using nontraditional experiences for practicing nurses. These experiences come in the form of direct and indirect care experiences in which licensed nursing students engage in learning within the context of their hospital organization, their specific care discipline and their local communities.
As the country focuses on the restructure of the U.S. health care delivery system, nurses will continue to play an important role. It is expected that more and more nursing jobs will become available out in the community, and less will be available in acute care hospitals.
1.Write an informal presentation (500-700 words) to educate nurses about how the practice of nursing is expected to grow and changes. Include the concepts of continuity or continuum of care, accountable care organizations (ACO), medical homes, and nurse-managed health clinics.
2.Share your presentation with nurse colleagues on your unit or department and ask them to offer their impressions of the anticipated changes to health care delivery and the new role of nurses in hospital settings, communities, clinics and medical homes.
3.In 800-1,000 words summarize the responses shared by three nurse colleagues and discuss whether their impressions are consistent with what you have researched about health reform.
4.A minimum of three scholarly references are required for this assignment.
While APA format is not required for the body of this assignment, solid academic writing is expected and in-text citations and references should be presented using APA documentation guidelines, which can be found in the APA Style Guide, located in the Student Success Center.
This assignment uses a grading rubric. Instructors will be using the rubric to grade the assignment; therefore, students should review the rubric prior to beginning the assignment to become familiar with the assignment criteria and expectations for successful completion of the assignment.
You are required to submit this assignment to Turnitin. Refer to the directions in the Student Success Center. Only Word documents can be submitted to Turnitin.
NOTE! NOTE! NOTE!
This question is split into two segments. i.e 500-700words and 800-1000 words. Please answer this question as itemized above. The references and citations should be between 2009 to date. If you don't understand anything about this NOTES, please let me know. Make sure the writers read this question very well before they answer it. There should be no rushing this time around, because I don't want to be complaining all the time. Please do a good job. I also intentionally asked for 1650 words so that you can split it into two segments as indicated above according to the question as covered by the amount/price.

Evolving Practice of Nursing and Patient Care Delivery Models
Q .1 Informal presentation
Nurses play a significant role in the healthcare delivery system in the United States. Recently the United States government restructured its healthcare delivery system with the introduction of the affordable care act. Consequently, the affordable care act opened up more nursing jobs across the states. The affordable care act also brought many other revolutionary changes. These changes are aimed to enhance the number of people accessing quality healthcare, reducing the cost of healthcare and overall improvement in patients’ experience. These changes are as a result of many driving changes in healthcare. These driving changes are accompanied by new nursing guidelines.
Through a continuum of care, the affordable care act aims at patient-focused teamwork. Nurses are expected to form an interdisciplinary team that will work for all people in the society including long term care facilities and the homeless. It is expected that long nurses in long-term care facilities will have to work under pressure since they are expected to giver care for lifelong patients. One of the ways in which the practice of nursing is going to change is through a medical home model. Medical homes in the affordable care act are supposed to provide quality healthcare at a reduced cost. Nurses in these medical homes should use an innovative approach to delivering affordable and quality care. Therefore, nurses and other specialists under extended time with patients are expected to meet patient’s needs. In the future, most people will prefer taking patients to medical homes than the conventional hospital setting. This model mainly targets patients with chronic diseases.
Liu, & D'Aunno, (2011) argue that in medical homes, teamwork is expected to play a critical role in the delivery of care. Nurses must be ready to work as team members. Furthermore, as a way of improving quality care, nurses are expected to have a personal and ongoing relationship with their patients. Therefore, nurses should be ready to come up with strategies that will be efficient for all patients in their homes. They should learn new strategies of giving care to patients in a medical home model. Nurses should form models that bring together nurses patients and family members that identify key issues in patients. It is expected that changes in nursing practices will increase the number of communal care centers.
Churchman & Doherty (2010) suggest that one of the factors that have increased mobility of nursing facilities is the formation of Accountable Care Organization (ACOs). Through Accountable Care Organizations, it is expected that many nurses will form networks through which they will use to deliver care to patients. Through coordinated services, many nurses are expected to offer specialized care in a coordinated manner. They must also be ready to perfect in their roles as specialized nurses. This means that nurses will now have a shared responsibility. Nurses are expected to be creative and innovative in order to offer Specialized and customized nursing care. Future nurses are, therefore, expected to prepare for these changes and ensure a smooth coordination in their networks and offer quality healthcare for Medicare patients.
Another significant group of nurses that will be affected by healthcare reforms is the nurses in nurse-managed clinic (Hansen-Turton, et al. 2010). It is expected that nurse-managed facilities will increase the use of evidence based practices to improve patient care as well as nurse satisfaction. Nurses in many nurse-managed facilities will receive support from the government to provide service and education to patients. With funding from the government, nurse managed clinics are expected to serve more people than before. The sustainability of nurse-managed clinics is expected to increase.
With increased literacy levels among many citizens in the US, more patients will demand safe and quality services from nurse-managed clinics (Duffy &Duffy, 2013). Quality and safety in the practice of nursing has been given a top priority. Many patients are aware of their rights as patients. In addition, many states in the United States have created laws that emphasize on safety and quality from nurses. There are already healthcare centers in the United States that keep track of nurses’ performance. It is expected that more and more healthcare facilities will develop databases that will monitor the quality services offered by nurses. Despite all changes, nurses remain primarily as caregivers. However, they are expected to overcome this traditional role and be innovators (Hassmiller, 2010). They also need to be culturally wise.
Q. 2
With all this in mind I want to share these insights and seek responses from nurse colleagues on these matters affecting nurses today. Nurses are expected to give care and interact with people of diverse religion, gender, sex and race. They are expected to work hand in hand with scientific researchers and social workers to advocate for the healthcare needs of the vulnerable members in the society. After sharing my presentation with nurse colleagues in my department, most of them were quite encouraged to learn that the government was committed to improving healthcare conditions in the country. Most of these nurses were happy to learn that the new law shall expect them to coordinate and help each other in dispensing their activities. there were a few of them who were not impressed by the new law since they argued that they will be forced to spend much time at their workstations that it is at the moment. The other thing that the colleague nurses were happy about was the increase in the financial allocation that came with the new law, which shall enable them to work with improved equipments to serve the patients properly. The other appealing aspect to them was that they will be moving in the communities and medical homes to discharge their duties other than being stationed in a single environment. There was this one nurse who impressed me much by arguing that this new law will help to improve the quality of healthcare thus reducing the number of deaths among patients. This was in line with my personal vow to help improve the quality of healthcare by doing my best as a professional nurse.
Q .3 Summary of responses from colleagues
Most of the nurse colleagues acknowledge that nursing practices have significantly changed in the recent past. The first colleague for instance, mentioned tha...
