The role of nursing is fast evolving since the nurses are being entrusted with a wider level of health care tasks. Taking care of the sick is becoming more complicated, yet most hospitals are understaffed under tight budgets. Due to the increment of some diseases such as obesity and diabetes, among others in the world population, it is clear that the system of health care is experiencing some shake-ups that need adjustments (Ridge, 2011). Efforts to improve nursing practice are underway and there is need to educate nurses concerning the way it is expected to grow and change.
Part One-To Educate Nurses
The change and growth of the nursing practice is very crucial so that heath care system can be able to keep up with the rising complications of illnesses. Among the most important concepts in health care system, the continuity of care is usually experienced by clients in the entire continuum. The problem behind inadequate continuity of care can be derived from the shortage of staff, complexity and acuity of health care requirements of clients, elevated demand for services, and limited experience in nurses, among others (Cheater, 2009). In order for this area to improve, the access to care is expected to improve and observance to prescribed treatment and screen also should improve. Moreover, the unidentified issues also needs to be recognized, fewer hospitalizations and improved satisfaction in clients, among other measures. The core and focus of the entire registered practice of nursing is expected to offer care to clients. Through the employment of the nursing procedure of assessment, planning and diagnosis, evaluation and implementation, these professional nurses have the skills and competency to offer holistic nursing care in communities, families, individuals and the entire population. The medical home evolved to comprise of primary care practice, which resulted to organizational models. The organizational models allow provision and coordination of wide-ranging care that is founded on well-built relationships between provider and patient. Therefore, there is a great emphasis that medical homes ought to be community based inter-professional teams, which include nurses, physicians as well as other healthcare professionals.
