Proposed Solution: (a) Describe the proposed solution (or intervention) for the problem and the way(s) in which it is consistent with current evidence. Heavily reference and provide substantial evidence for your solution or intervention. (b) Consider if the intervention may be unrealistic in your setting, if it may be too costly, or if there is a lack of appropriate training available to deliver the intervention. If the intervention is unrealistic, you may need to go back and make changes to your problem statement before continuing.

Organization Culture: Explain the way(s) in which the proposed solution is consistent with the organization or community culture and resources.

Expected Outcomes: Explain the expected outcomes of the project. The outcomes should flow from the problem statement.

Today, adults who are over 18 years are vulnerable to getting Type 2 Diabetes. Type 2 diabetes is a medical condition that raises the blood sugar levels of an individual. When signs and symptoms are recognized earlier, it is possible to get the right treatment. This reduces the risk of severe complications. The adversities associated with this group include frequent urination, increased thirst, fatigue, blurry vision, and pain in the hands and feet. To deal with this problem, the best intervention method to be used include structured, intensive education maximizing engagement, motivation, and long-term change. Compared to the normal treatments, this intervention method will be helpful because the number of young individuals who have been diagnosed with the disease is increased worldwide (Frost, 2017). This intervention method will improve control and reduce complications, costs and illness burden. The intervention can be delivered in four group sessions for 6 to 12 months. Structured, intensive education maximizing engagement, motivation, and long-term change is a realistic intervention towards lowering the cases of Type 2 diabetes among young people. However, a particular department in the hospital should be charged with the responsibility of enhancing the safety of the patients.
