1 What did you learn from the news reports?
AAP News (2020) released an article titled ‘How to code for chronic care management of patients with one disease.’ It discussed the basic information on how coding is done for chronic healthcare conditions. Some of the codes are about specialists and sub-specialist care providers. The codes relate to care plan oversights (99339, 99340, 99374-99380), care management services (99487, 99489, 99490, 99491), and transition care management (99495-99496). The first two are for one and two chronic diseases, respectively. The transition care is reported for clients who have around 29 or more days after they were discharged.
There was a challenge in permutating the codes, hence developing common letters like ‘G’ for medicare. So some codes were generated from the same letters and shared subsequent number...
