The focus of my scenario is discharge planning and how the transition for patients can be made better. While patients are being discharged and released to other levels of care, many mistakes are likely to happen if there is no proper communication between the different levels of care. However, the communication that needs to take place has to involve relevant data regarding the patients to help ensure that their next level of care is seamless. For example, suppose a patient is being discharged from hospital care but has to transition to physical therapy. In that case, there is a need for the therapist to have all the data they need so that they may not interfere with the progress already made by the patient. The nurses and physicians have to provide as much data as possible to make the transition easier for the following professionals.
The data that is needed and that must be shared during discharge planning mainly entails the type of care a patient still needs, suggestions of those who can offer that care, the needs of the patient, the meals to be given, and maybe the changes that could be made at home (Soong et al., 2013). It is also crucial to share the medication that a patient has been taking while in the hospital. This data should be collected by the nurses in the hospital and then shared during the transition period. Nurses ought to make the experience of the next caregivers easier for the benefit of the patient. The more the data made available, the higher the likelihood that there will be increased professional collaboration. As more professionals collaborate, patients stand a better chance of survival. However, collaboration increases with an increase in data/information sharing.
