Overview of assignment
The nurse-sensitive quality indicators and nursing quality indicators are relevant to determine the effectiveness of the nursing interventions and are useful in quality monitoring and improving the outcomes of nursing interventions. The National Database of Nursing Quality Indicators (NDNQI) helps in quality improvement, supporting efforts to advance patient care and achieve high-quality care (Owens & Koch, 2015). The other indicators are “Nurse-Sensitive Service line/Unit -Specific Indicators”, “Nurse-Sensitive General Indicators” and “Nurse-Sensitive Patient Satisfaction Survey Indicators”. Using the nursing data indicators is beneficial to identify areas requiring improvement based on the quality outcomes. The accuracy and reliability of the nursing data and the quality indicator is important in informing the nurses and other health care workers on areas that require improvement. Data collected on the dashboard analysis is used to develop a nursing plan and is useful to identify interventions required in the prevention of pressure ulcers.
Analysis of the data and description of the performance
The data were collected for an inpatient rehabilitation unit for four quarters with information on the nurse service quality indicators, the mean values, target values, the actual results, and the variance. Negative variances indicate that there is a need to improve nursing intervention to improve the quality of care and health outcomes.
The area of performance selected is pressure ulcers taken from the NDNQI and the indicators include the “Patients with pressure ulcers”, and “Patients with acquired pressure ulcers” metrics. Pressure ulcers occur when the skin and underlying tissues in certain parts of the body are constantly pressed, and especially, for patients who stay in the same position for long.
In Q2FY09 the % patients with pressure ulcers was 20.00 well above the target of 2.84, but the % patients with acquired pressure ulcers in the same quarter was 0.00. Additionally, in the quarters, Q3 FY09 and Q1 FY10 there were patients who acquired pressure ulcers while hospitalized. It was only in Q4 FY09 that there were a 0 % of patients with pressure ulcers, but there was a general decline in the rate of patients with pressure ulcers.
Nursing plan and best practices
Identifying the factors associated with the risk of pressure ulcers for both tissue tolerance and pressure is necessary to improve the patient outcome. Skin integrity assessment is one strategy to prevent pressure ulcers and implement the most effective prevention strategies, identifying the patients at risk of pressure ulcers help to reduce the risk of pressure –related injuries.
