Strong government push in the 2000s resulted in hospitals and physicians increasingly use the electronic medical record (EMR) or electronic health record system (EHR), with Health Information Technology for Economic and Clinical Health Act (HITECH) and ACA are some of the legislations, which accelerated the adoption of health information technology. The use of EMR/EHR and patient portal in chronic care allows better coordination of care in health team (Furukawa et al., 2015). The collection, management, processing, transformation and communication of information with the patients, family or community is necessary in nursing practice and health information technology can help to achieve this. The challenges with dealing within EMR/EHR and patient portal include weak functionality and interoperability, workflow disruption and high costs, but the technology is preferred for providing access to accurate patient information, enhance communication, portal feedback
There are various electronic medical and health record platforms, including those that are customize for various health institutions. However, in adopting the technology the functionality of these platforms need to be carefully assessed to ensure that they are compatible for the hospitals and the critical care team. This is one of the challenges that affects the successful implementation and adoption of the technology. Interoperability of health devices and technology is prioritized as it improves patient safety and is useful in gathering useful evidence especially when there are large data sets, which makes it easier to identify unusual data and alert the health practitioners (Nguyen, Bellucci & Nguyen, 2014). Nonetheless, the various devices do not necessarily communicate well with the others even as interoperability minimizes delays as the information is well integrated and comprehensive.
There are cost and security issues, which pose a challenge to implementing the most effective health information technology. The EMR, EHR and portals are at times are fragmented and the security of data in electronic devices across the system affects the health practitioners provide care. This has a negative impact on the patients and providers, and when the costs are high this affects the quality of care. There are also unresolved costs issues associated with integrating new technology in health care. Yet in critical care adopting the latest technology that is customized to meet patient care needs makes a big difference in the health outcomes especially when there is minimizing delays in communication when there is seamless integration and access to patient data and information.
