Introduction
Over the years people have criticised the health care system because it lacks to provide the continuum of care for patients suffering from chronic diseases (Pieber, 2012). The affordable care act of 2010 seeks to put to an end the primary care provided for patients and promote a patient-centered system based on medical homes. If properly implemented, the act has the capability to create better opportunities for the creation of a patient-centered system. It is essential for health institutions to operate within this act and provide the continuum of care to patients (Cutler, 2015).
Purpose of the program
The purpose of this program is to ensure the provision of long-term care for patients suffering from long-term illnesses. The program will create a platform and make sure that counsellors are available around the clock for patients. It will also ensure that people with chronic diseases are treated fairly and at a reduced cost in the hospital setting.
Target audience
The target audience for this program will be people suffering from long-term diseases. Over the years people have developed certain stereotypes about long-term illnesses. Some think that seeking treatment is just a waste of time and resources because they will eventually die. This cannot be further from the truth because the reality is many people have fully recovered from these diseases. The creation of this program will create a space where this person can meet and discuss the problems affecting them as a group. The main target population for this program is those suffering from chronic illnesses.
Benefits of program
This program will be beneficial to both the patient and the hospital. For the hospital, it will enhance credibility for the institution because people will believe that the hospital cares for them. The provision of the continuum of care will create a good image for the hospital. This is beneficial because it will lead to an increase in patients which automatically translates to increased profits (Parekh, & Fang, 2014). As for the patients, it will play a significant role in prolonging the lives of patients. This program will use an approach which deals with the cause of the problem. In some cases, people suffering from these diseases have something that triggers the disease. Understand what triggers it and dealing with the problem from there will be beneficial towards improving and prolonging life.
