Background
The increasing rate of diabetes among the overall population in the past few years has resulted in a significant health concern that needs to be urgently addressed. Despite the appropriate measures that are taken to control diabetes, the disease has continued to weigh so much on the patients and their families due to the high cost of disease control involved. Also, the disease affects the community at large because diabetes patients cease to become more productive in their places of work due to their health condition. According to the new reports, more than 100 million people are suffering from either diabetes or prediabetes based on the CDC data (Kirkman et al. 2012) There have been efforts to contain the disease from all sides of the medical field. However, the efforts and energy have only gained little benefits. Diabetes does cause not only serious health issue to the patients but also cause financial strain to the family of the patient. That means that there is a need to come up with effective ways to control the rising rate of diabetes cases.
Problem Statement
Diabetes is a critical health concern that needs a lot of resources and attention to be solved. The rate of getting this chronic illness is on the rise due to the change in the lifestyle of human beings in recent years. It has been predicted that the disease will even increase further at an alarming rate in the future. The effectiveness of how well the diabetes disease is managed not only to depend on the medication and but also depends on how well the patient can follow the medication instructions and the prescription. The problem is that many of the patients do not have enough knowledge on how to use these medications and they end up misusing it which renders the drug ineffective. With that said, it is, therefore, essential to developing a strategy on how the patients can be educated on how they can use the medication for the therapy to be effective.
Purpose of the Change Proposal
The primary aim of the change proposal is to come up with more effective ways of diabetes management by enlightening the patients about the right way to use their therapy and medication. The objective is very crucial especially a time like this when the disease is becoming a more prevalent disease in the country and also globally. Another purpose of the proposal is to give knowledge to the target people about how diabetes can best be managed and controlled. Lastly, the change proposal aims at influencing the policies and the policy-making process by giving the relevant departments the insight into countrywide strategies and plans of diabetes control.

Literature Search Strategy Employed
The primary search strategy which was used for coming up with the literature review is the search engine method which included the library search, Cochrane, medicine databases, and google scholar. The two search engines are rich with plenty of scholarly articles and journals which searched to be easier and successful. The keywords were used to select the relevant educational materials from the database. The keywords used to search for the literature included terms such as diabetes and therapy treatment. After several articles and journals were obtained, an article and the journal review was conducted to examine the extent of the relevance of other journals to the research topic.
Evaluation of the Literature
The prevalence of diabetes disease has been on the rise globally and continues to affect many families in a negative manner such that it has been declared a severe health issue. The problem has attracted many stakeholders including the government agencies, the policymakers, and the entire community at large. Due to the nature of the problem and the many numbers of people it has affected, many researchers have researched and examined several factors relating to the ineffectiveness of the diabetes therapy or medication.
