The conceptual model that follows should be used as a guide in identifying the determinants or factors that contribute to the observed problem.Hint: When reviewing this model, begin at the individual level rather than the local, regional, or global levels, and work outward to identify relevant contextual factors.

From this model, please identify and discuss four contextual factors relevant to your problem, and note how each either diminishes or exacerbates the problem you are seeking to address. Your overall response should be 4–5 double-spaced pages, excluding references. The increased prevalence of diabetes among the youths is a concern to health practitioners, and self-care practices improve the disease management and health outcomes. Diabetes patients with knowledge on the management of the disease are more likely to have a positive attitude towards self-management practices. Additionally, satisfaction of the social support promotes positive health behaviors, as parents, family members and peers offer emotional support. This may extend to the role of spirituality where communities support positive coping strategies. Availability of resources improves preventive care, while making it easier to manage diabetes since individuals can access better care and the most appropriate information. Even though, patient education helps to improve the patients, well-being those who have positive attitudes and beliefs are more likely to support self-management practices. Hence, it is necessary to consider patient awareness, social support, and availability of resources, patient attitudes and beliefs as contextual factors that influence diabetes self-management.
Addressing the barriers to diabetes self-management is necessary to improve the health outcomes of adolescents in the short-term and long-term. Adequate self-care management practices empower the patients to adhere to the recommended. Knowledge on diabetes self management not only improves understanding of the diseases, but also improves glucose monitoring, proper dieting and engagement in physical exercises. Knowledge and literacy on diabetes management promotes self-management practices. Adolescents live under the care of their parents and guardians, and inadequate counseling and emphasis on diabetes self-management means that it is difficult for them to assimilate information compared.
The case for increasing diabetes knowledge is to provide essential skills and empower adolescents with diabetes to be actively involved in improving self-management. This is associated with improved dietary habits, blood sugar levels monitoring in the short-term nonetheless, there is no guarantee that in the long-term knowledge of self-management will be associated with gylcemic control. Collaboration of the adults and the health care practitioners is valuable in empowering the diabetes youths. Emphasis on lifestyle factors during training not only improves self-management practices, but also improves the ability of people with diabetes to control the condition (Aloha, & Groop, 2013).
Significant social and emotional support helps the adolescents cope with diabetes and adapt when facing stressors. Equally, lack of social support may result in negative health perceptions as individuals are likely to neglect self-care. The role of family members then cannot be ignored since they are trusted to care for the person where they feel that they are valued. Social support is a sociocultural variable that influences health behaviors, especially when the diabetic patients are satisfied that they are valued (Aloha, & Groop, 2013). For instance, the close friends and family members offer support to ensure that the adolescents with diabetes follow the recommended dietary and exercise routine.
Close family members give reminders and provide emotional support that the young diabetic youth can persevere and follow recommended plans. Social support can address the barriers to self-management, since family members help to access health care. Minorities and immigrants face more challenges to self-management when there are few culturally appropriate resources. Lack of social support makes the individuals vulnerable in self- management practices because of inadequate supportive networks. Parents support their children’s adherence to diabetes management, providing instrumental and emotional support as they are responsive to the teenagers’ needs.
