Introduction
As the director of operations at Centra Health Lynchburg, VA, I have noted the need to make improvements in the medication reconciliation processes. According to Patel, Pevnick & Kennelty (2009), medication reconciliation ensures a patient's medication list is complete across the transition process. Centra health operates in a dynamic and competitive environment which high-quality services. The current medical reconciliation process is ineffective in reducing discrepancies in patients’ medication list. This policy proposal seeks to improve the current medication reconciliation processes to improve patient experience at Centra Health.
Proposed Policy Change
The policy proposes a standardized approach to medication reconciliation across all the departments. This will ensure that departments within the organization work together to harmonize patient medication lists. Secondly, the policy proposes the acquisition of digital tools to enhance Electronic Health records (EHR). Data obtained from the EHR system will be used to guide healthcare staff on the updated medication lists. Thirdly, the policy recommends the facility to establish a multidisciplinary team consisting of different healthcare staff. This will be critical in implementing the new changes to enhance the medication reconciliation process.
The implementation of the proposed change will benefit the hospital in the following ways. Firstly, it will improve patient safety. When medication lists are accurate and updated at all the transitions of care, this will reduce the risk of medication errors like omissions, duplications, and dosing errors. A robust medication reconciliation process serves as a safeguard, ensuring safety at all stages of care (Soon et al., 2021). Secondly, enhanced medication reconciliation will improve operational efficiency in the organization. Healthcare providers will spend less time reconciling medications during patient admissions, transfers, and discharges.
