The stakeholders that were associated with the problem include the Baxter Health Corp, the manufacturers of the heparin. The company redesigned the vials but did not recall the old vials which in turn brought about the confusion regarding the concentration of the drug (Kroft, 2008). The pharmacy technicians who delivered the drug to the pediatric unit without properly checking the concentration of the vials were also stakeholders associated with the overdose. The nurses also administered the drugs without properly reading the label on the vials first (Ornstein, 2008).
As a result of this error, the twins could have lost their lives through excessive bleeding. In addition, by leaking the information to the press, the hospital staff involved violated the privacy rights of those infants (Ornstein, 2008). The family now had to deal with the media, on top of what was happening to their infants. In addition, the hospital's image was damaged as a result since the state investigation revealed that there had been other similar cases, indicating negligence on the part of the hospital.
However, a few actions were taken to mitigate the risk of such an occurrence. The major one was that Baxter Health Corp withdrew all its heparin drugs from the market since it had brought about the confusion in the first place. Withdrawing the drug means that the confusion that was created by the redesigned vials was eliminated.
