CD is a digestive disorder that causes damage to the small intestine. While many patients with the disease remain undiagnosed, it is estimated that it occurs in about 1% of the population (Green & Cellier, 2007).
Causes of CD
CD is caused by interaction between gluten ingestion, genes, immune responses, and environmental factors (Green & Cellier, 2007). Gluten derived from barley, wheat, and rye is poorly digested in the upper gastrointestinal (GI) tract. The gliadin from the gluten has a number of toxic components. Immune responses to gliadin trigger inflammatory reactions. Genetics is also responsible for CD as there is the involvement of leukocyte antigen region (HLA) in 95% of patients (Banerjee, 2010). Environmental factors associated with CD are protective effect of breastfeeding (Green & Cellier, 2007) and consumption of wheat and related cereals (Banerjee, 2010).
Risk Factors for CD
Some of the risk factors associated with CD according to Banerjee (2010) include:
* Autoimmune thyroid disease
* A family member with the disease
* Type 1 diabetes mellitus
* Down’s syndrome
* Dermatitis herpetiformis
* Immunoglobulin (IgA) deficiency
* Addison’s disease
* Primary biliary cirrhosis, and
* Inflammatory bowel disease
Symptoms of CD
Common symptoms for CD include GI distress and weight-loss (Denton, 2018). According to Banerjee (2010), infants who are affected by the disease develop swollen abdomen, diarrhea, and vomiting. As described by the author, these infants are generally irritable and unwell. In children between 2 and 10 years, there may be minor GI symptoms, delayed growth and puberty, nutritional deficiencies, and anemia. In adults, while there are less frequent GI symptoms, classical symptoms of abdominal bloating, weight-loss, and diarrhea are common (Banerjee, 2010).
