Approach to care
Following the patient’s dire condition, a care plan is developed on the assumption that the patient will be admitted to an inpatient facility. Dizziness, inability to stand physical exercise, loss of appetite, respiratory challenges and fatigue are common among patients exhibiting CHF symptoms. The approach to care plan intends to prolong the patient’s ability to withstand physical activity. Mr. P is subjected to bed rest to reduce cardiac activity. The bed rest also enhances Mr. P’s oxygen consumption. Mr. P’s wife is required to provide emotional support as a way of improving the patient’s respiratory activities. Mr. P is elevated to a Fowler’s position to improve aeration. The position enhances the patient’s respiratory difficulties. The care plan also focuses on the patient’s excess edema. Mr. P is put on digitalis to increase the cardiac output and diuretics to collect edematous fluid. Mr. P requires constant observation and monitoring by the caregivers.
Treatment plan
Mr. P will be subjected to a multidisciplinary treatment plan aiming at achieving cardiac rehabilitation. The rehabilitation therapy aims at assisting the patient to recover from the life-threatening condition and resume normalcy. The program includes creating disease awareness to both the patient and his caregiver, engaging pharmacologic therapy and administering the necessary polypharmacy and offering cardiac intervention (Gulanick and Myers, 2014). Aggressive cardiac rehabilitation is essential to assist the patient to achieve and retain the required health both physically and emotionally. The plan involves reducing myocardial infarction, risk factors, managing stress, promoting a healthy lifestyle and physical activity, advising on healthy CHF-friendly foods as well as conducting regular checkups and assessments (Greenberg and Hosenpud, 2007). The initial evaluation will target Mr. P’s ability to withstand physical activity and light exercises. Evaluation results will guide Mr. P’s immediate and future treatment plans.
