You will need to download medical apps onto an iPhone, iPad, or Android device to complete this assignment. If you do not have any of these devices, you may use the app in a web browser on the host’s website. Step 1: For this assignment, choose one disease process/condition that you see most often in your practice. The goal is to create a tool kit item for your use as you educate your patients.   Step 2: Choose two (2) patient-focused medical applications. When choosing these applications, put yourself in the role of the patient. What applications would help you? What applications might you download to your smartphone? Why?   Step 3: Create the first concept map for patient-focused medical applications. Choose three (3) criteria from the list below which you used to select the chosen application. Explain how each of the chosen (3) criteria are addressed in the application. Inform: Provide information in a variety of formats (text, photo, video)  Instruct: Provide instructions to the user  Record: Capture user entered data  Display: Graphically display user entered data/output user entered data  Guide: Provide guidance based on user-entered information (e., recommend a health care provider or course of treatment)  Remind/Alert: Provide reminders to the user  Communicate: Provide communication with the user and/or provide links to social networks.  Briefly describe how the application meets the criteria in each of the concept map nodes/sections. Do not leave the box blank with just the prompt – there must be a description.  Example: Instruct: This application provides a variety of teaching guides for the patient to review. Also, there are links to other sites to find more information.    Step 4: Choose one (1) provider-focused medical application. When choosing this application, think about being the provider (nurse, physician, advanced practice nurse). What application would help you? What application might you download to your smartphone and why?   Step 5: Create the second concept map for provider-focused medical applications. Choose three (3) criteria from the list below which you used to select the chosen application. Explain how each of the chosen (3) criteria are addressed in the application. Inform: Provide information in a variety of formats (text, photo, video)  Instruct: Provide instructions to the user  Record: Capture user- entered data  Display: Graphically display user entered data/output user entered data  Guide: Provide guidance based on user-entered information (e.g., recommend a health care provider or course of treatment)  Remind/Alert: Provide reminders to the user  Step 6: Using the Galen library, research at least two (2) scholarly articles related to medical applications.  At the bottom of your concept map template, write a summary (at least two paragraphs*) about your applications and include the following:  Provide a brief explanation for the chosen topic and how it applies to your practice.  Explain how the research supports the use of medical applications in practice.  Explain which sphere of care from the AACN Essentials the chosen applications best fit: Wellness and Disease Prevention, Chronic Disease Management, Regenerative/ Restorative Care, or Hospice/ Palliative Care. What changes would need to be made to use the selected application in each of the other 3 spheres of care? (Use the NIH as a reference for evaluating health information).
