Mark is a 19-year-old university student who last year started experiencing increased difficulties in terms of mental health, including severe depression, social isolation, and anxiety. Specifically, self-harming behaviors have increased with academic decline after the death of a friend who was very close to him. This mental health assessment and care formulation seek to work out an involved care plan with referral to Mark's synopsis through a holistic-client-centered treatment model. It will involve a multi-disciplinary care team to address the care needs that relate to Mark's mental health needs and emotional and social needs. In addressing his treatment, the biological, psychological as well as social aspects have been considered together with family and peer support to ensure that the needed support is provided to help the treated individual recover and be resilient. This approach of intervention will not only meet Mark's current urgent needs. Still, it will also help him to get long-term solutions that will help him overcome his mental health problems and reintegrate into school and society.
Current Context of Care
Mark is a 19-year-old male who is presently an inpatient in a mental health facility with a diagnosis of severe depression and anxiety. His symptoms have worsened after the loss of a friend, and as such, he has been experiencing complications emotionally and psychologically. Specific symptoms present in Mark's presentation include pervasive sadness, anhedonia, self-preoccupation, and social isolation, including episodes of self-harm and regular anxiety attacks. Furthermore, his mental status has also been characterized by sleep and appetite disturbances, which has led to his deteriorating state of health. The current plan of care for Mark is focused on the need to call his psychiatrists, clinical psychologists, and social workers regularly. The intervention strategy entails pharmacotherapy and psychotherapy as the medically recommended course that will assist in managing his moods leading to depressive episodes. The clinical team is trying to evaluate and optimize the doses of the antidepressant/anxiolytic medications the patient receives to maximize the therapeutic outcomes and minimize side effects.
Cognitive Behavioral Therapy (CBT) is being practiced in individual therapy to enable Mark to face and replace his negative thoughts and emotions. This therapy aims to challenge the negative thoughts and suicidal depression of guilt and self-blame for the death of his friend, as suggested by Young et al. (2021). Ideally, therapy involves working on new strategies for interacting with others and improving the client's personality. Social support is an essential element in Mark's current care plan. He is now being encouraged to go back to his relatives and friends to get someone to support him emotionally and or even physically. They are also actively trying to introduce family therapy to facilitate or observe the relationships and improve Mark's support at home. Multi-disciplinary intervention includes quality structured activities and approaches aimed at diminishing Mark's isolation of Mark and encouraging him to get involved in social and leisure activities, which have been shunned in the past. The idea is to assist him in regaining his social relationships and practicing activities that he once used to do, which would greatly assist him in his healing process.
Consumer Goals for Recovery and Future Plans
Mark's goals in rehabilitation involve his current psychiatric state, what he wants to achieve in the short and distant future, and his mental health improvement while enhancing functionality. The primary purpose is mood stabilization and reduction of the severity of depressive symptoms that Mark is suffering from. This entails having a strict look at his current medication regime and proactively creating changes where desirable due to a change in his progress. Psychiatric follow-ups will be routine, and any changes in Mark's behavior will be detected to help his doctor respond to new developments. In addition to this, another significant component in Mark's recovery strategy is Cognitive Behavioral Therapy, also known as CBT, which aims at changing and disproving the negative thought processes that Mark experiences.
Another important objective is social reintegration, which involves contact with the social surroundings. The isolation has served to worsen his mental health problems, which makes rebuilding his social circle part of the recovery plan.
