Illness is a challenging time for the patient and the family. The feeling of hopelessness results in psychological distress for the family members. They may feel worried, upset, frustrated and guilty because of what their relative is going through. The emotions differ depending on the severity of the patient. Similarly, patients, especially parents, are worried about their children's condition. Illness increases the financial cost to the family. Family members may increase their working hours to support the sick members. As nurses handle patients and family members, they must be aware of the challenges they face as they undertake the care coordination process.
Strategies for collaborating
A strategy for collaborating with patients and families is patient/family-centred communication. Such kind of communication acknowledges the whole person, considering their background, personality and other relevant factors. When passing information to patients and family members, nurses should be careful to avoid hurting the emotions of their clients (Jiménez-Herrera et al., 2020). Timely and unbiased communication allows patients and family members to be more proactive in decision-making. Another strategy is offering culturally competent care. Change management allows healthcare facilities to transform workflows seamlessly. An aspect of change management is the collaboration between the management and the nurses. Formulating a smooth workflow requires the management to work with healthcare professionals, especially nurses. The management is responsible for formulating policies, while the nurses are required to implement them (Arabi, Rafii, Cheraghi & Ghiyasvandian, 2014). The collaboration between the two groups will ensure that a healthcare facility adjusts its processes to meet the needs of the patients and their family members. The management will understand the nurses' concerns, while the nurses will work towards fulfilling the formicated processes. The collaboration between the management and the nurses will ensure that patients and their families receive the best possible care.
Additionally, nurses need to enhance the safety of the patients. In the hospital, patients are at risk of getting healthcare-acquired infections (Ali et al., 2018). Such diseases can increase the length of admission and hospital costs. Sometimes, infections can lead to death. Nurses are responsible for ensuring that patients do not get new infections while in the wards. They need to take measures like hand hygiene when handling patients. Moreover, nurses must clean the wards and surfaces to avoid spreading infectious diseases. Patients, especially older ones, are at an increased risk of falling. Patients who fall can be injured, complicating their situation further. Nurses need to implement measures aimed at reducing patient falls. Working towards patient safety is an important aspect of change management that should be considered.

Ethical decision-making is an important factor in coordinated care. When working in a team, information sharing is important.
