Introduction
Doctor of Nursing Practice-prepared nurses are expected to be instigators of change for the betterment of patients, practitioners, and organizations. The ability to identify and plan for ways to use change to mitigate various risks is critical to improving outcomes for everyone in the healthcare environment. In this assessment, you will demonstrate this by completing a risk mitigation plan and an explanation of your plan.
Preparation
Note: Remember, the assessments in this course build upon one another. Also, keep in mind that your findings from the SAFER Guides will be used to inform your later assessments.
In the previous assessments, you observed opportunities to use technology/informatics to make practice changes, reviewed organization or provider-type performance, and identified real or potential risks associated with the technology/informatics-driven practice change.

If you choose to make revisions based on feedback from a previous attempt, you should highlight your revisions in yellow. For example, if you made revisions from attempt one and would like the instructor to review the content when grading attempt two, the content needs to be highlighted. Therefore, the instructor reviews only the content highlighted on attempts two and three. If the entire paper is highlighted, the paper will be returned ungraded and will count as an attempt. Track changes are not a substitution for highlighted text.

For this assessment, you will create a 4-page paper discussing your mitigation plan for the recommended technology that you chose for the previous assessments. You will start your paper with an introduction providing a summary of the patient safety concerns and a summary of the previously recommended technology. Then, you will use your SAFER Guides findings and resources to create a risk mitigation plan.
Overall, your assessment will be assessed based on the following criteria and should be presented in the order listed:
Explain your risk mitigation plan.
Use the Risk Mitigation Plan Table Template [DOCX] to list the risks identified by the SAFER Guides, the potential occurrence of the risk (frequent, sometimes, never), the potential for harm [(patient or organization) (severe, mild, none)]. Add the table in this template as a properly formatted APA appendix at the end of your assessment.
In the body of the paper, include a narrative about the content in the table.
Explain any ethical or legal issues that could result from not addressing identified risks.
Think about not only what the potential ethical or legal issues, but also the impact on patients, practitioners, and institutions that could result from them.
Discuss how your recommended technology complies with the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
How will your recommended technology comply with HIPPA?
What measures are in place for cyber security?
Justify proposed actions to address identified risks.
Make sure you support your justification with cited support from scholarly peer-reviewed literature or evidence-based practice.
Identify change management strategies that could help others implement the proposed actions in their practice.

Emergency departments (EDs) work in settings of a high risk with a high velocity where the very fragmented information, a disruption in the workflow and the damaged connection with patients can pose a direct threat to patients. The operational size of the HCA Healthcare, which serves millions of patients every year, exacerbates these risks, especially when providers have to use multiple electronic health record (EHR) interfaces, address too many clinical decision-support (CDS) alerts, and resolve unfinished or late patient data (HCA Healthcare, 2025). False alarms, incompatibilities, and lost information continue to be the reasons behind medication errors, diagnostic delays, and avoidable injuries in the ED (Litchfield et al., 2025; Sittig et al., 2025). As a way of solving these issues, an AI based smart clinical dashboard has been suggested to combine EHR information, CDS results, medication safety signs and operational metrics into one real-time service. Although this technology has a high potential of improving situational awareness and decision-making, its use poses a great clinical, ethical, and legal threat when not well addressed. The SAFER Guides published by the Office of the National Coordinator of Health Information Technology feature a formalized approach of evidence-supported risk identification and mitigation in the process of health IT implementation (ONC, 2023). The present paper contains a detailed risk reduction program and mitigation approach, designed in accordance with the results of the SAFER Guide in order to facilitate the implementation of the AI-based smart clinical dashboard safely, ethically, and within the legal boundaries in HCA Healthcare emergency departments.
Risk Management Plan
Overview
Minimizing risks is a very important part of health information technology (health IT) implementation especially in the complex setting like the emergency departments. Risk mitigation is the prevention of harm to patients or organizational liability by proactively discovering and prioritizing system vulnerabilities and eliminating them. The SAFER Guides focus on a sociotechnical approach, acknowledging that the risks associated with safety arise in the interactions between technology and people, workflow and organization structure (Sittig et al., 2025). In the case of HCA Healthcare, the risk mitigation strategy is more of prevention and not a reactive correction strategy. By applying SAFER domains, such as system interfaces, communication with clinicians, CDS, contingency planning, and organizational responsibilities, the leadership can organize the discussion of the high-risk areas of failure and interrupt high-risk areas of failure before the deployment of the AI dashboard (ONC, 2023).
Identified Risks
Various high-risk factors were determined based on SAFER Guide analysis as listed in Appendix A. The failure to correctly monitor system interfaces is one of the significant risks that happen occasionally but with serious potential damage. A breakdown of the interface may cause the loss of lab results, medication orders or diagnostic imaging in the ED, which translates to delayed or incorrect treatment. Since emergency care is time-sensitive, any loss of data, recorded in a short period, may cause harm to patients and lawsuits against the organization. The other most common and egregious threat is alert fatigue and a lack of usability of CDS. The number of alerts with poor prioritisation, leads to cognitive overload and alert overrides, which weaken safety systems aimed at avoiding mistakes (Litchfield et al., 2025). This danger is increased in EDs where clinicians are required to receive a large amount of data under time constraints. The incomplete control over AI algorithms is also a risk that is hard to avoid and, unfortunately, is severe. AI-generated suggestions are subject to bias, inaccuracy, and inappropriateness to clinical use unless administered by supervision, and it is likely to introduce inequities in care (Freeman et al., 2025; Okwor et al., 2024). The unreliable AI results can directly lead to negative outcomes in emergency environments, where the decisions are usually made in a few minutes. Lastly, informal communication workarounds represent a common problem with moderately low to severe levels of occurrence and damage. These activities bring the possibility of miscommunication, information loss, and poor documentation, especially the case of the handoffs and transfers of patients (Livesay et al., 2023). Lastly, poor real-time safety analytics limits the capacity of the leadership to identify emerging risks and act promptly, postponing their correction and undermining organizational learning.
