Physiotherapy Management Case Name Course Institution Date Physiotherapy Management Case This paper focuses on a critical case discussion of the physiotherapy management of Matthew, a person with a complex condition. Matthew Stokes is a 20-year-old with cerebral palsy (CP) secondary to a traumatic brain injury at birth. He experiences recurrent chest infections, feeding difficulties, and challenges with positioning due to scoliosis and dystonia. Using Matthew’s case, the analysis will critically review the current physiotherapy interventions for people with complex and enduring health conditions. The analysis will extend to showcase the psychosocial factors that might influence practice when working with people with complex and enduring health conditions. Ultimately, Mathew’s case will be employed to evaluate strategies aimed at supporting individuals, families, and carers in the management of complex and enduring health conditions. Understanding Matthew’s Condition Matthew was diagnosed at birth with cerebral palsy (CP). CP is a non-progressive neurological condition affecting mobility and posture that is frequently triggered by a brain injury (Chang et al., 2023). Matthew's traumatic brain injury at birth set the onset of the difficulties. People with CP may have poor motor abilities, muscle stiffness, and other comorbidities (Costa et al., 2023). According to the literature, people with CP are more susceptible to respiratory problems due to altered chest mechanics, weak respiratory muscles, and an impeded cough. The other notable symptoms of this condition include learning disabilities, vision/hearing problems, and tremors/involuntary movements (WHO, 2013). The severity of CP symptoms can vary from mild to severe depending on the scope of the injury that the patient incurred. There is no treatment for CP. however, some interventions could help to improve the well-being of the patients by managing the symptoms. Some of such interventions include surgery, medication, speech therapy, occupational therapy, and physical therapy (Hou et al., 2023). Physiotherapy Interventions In this analysis, emphasis will be put on postural management and seating as the first physiotherapy intervention as well as hydrotherapy as the second intervention for Matthew. Postural Management and Seating Postural management and seating are critical components of the care plan for individuals with Cerebral Palsy (CP). Matthew's difficulty fitting into his chair raises concerns about posture. Hence, an in-depth review of Matthew's seating and positioning, perhaps involving an occupational therapist, is essential. Notably, Matthew's scoliosis and dystonia necessitate careful positioning to prevent contractures and optimize function. His current comfy chairs pose challenges due to inadequate support, potentially contributing to feeding difficulties and aspiration risk. Addressing Matthew’s postural management and seating issues could effectively trigger positive outcomes in pursuance of his well-being. There are specific objectives that the specialists will be pursuing by considering postural management and seating for Matthew. The first objective will be the prevention of musculoskeletal complications. CP often leads to muscle imbalances, contractures, and skeletal deformities (Alisha et al., 2023). Effective postural management aims to prevent these complications by optimizing alignment and distributing pressure evenly. The second objective is to enhance Matthew’s functionality. Proper posture enables optimal muscle function and movement patterns. Hence, by supporting alignment, postural management can improve the efficiency of movement and functional abilities in CP patients (Law et al., 2016). The third objective of this therapy is to enhance the patient’s respiratory function. Postural management influences respiratory mechanics (WHO, 2023). That is, poor posture can compromise lung expansion and ventilation, exacerbating respiratory issues commonly seen in CP. Maintaining an upright posture aids in respiratory function and clearance of secretions. There are more objectives relating to implementing seating therapy for Matthew. Primarily, Matthew requires a different wheelchair selection. Wheelchairs for CP patients should provide appropriate support, stability, and adaptability to meet their specific demands (Boyd & Galea, 2017). Features like tilt-in-space and reclining help to redistribute pressure, increase comfort, and control tone. Additionally, custom seating solutions, which include molded seating and positioning cushions, provide tailored support to accommodate individual postural needs while preventing asymmetries. These technologies help CP patients feel more comfortable, stable, and functional (Lof et al., 2016). Finally, the new seating interventions should foster dynamic seating strategies. Dynamic seating systems incorporate movement into seating, allowing for controlled shifts in position and absorption of forces during functional activities. This promotes active engagement, reduces fatigue, and prevents pressure injuries. To execute postural management and seating therapy efficiently, there are specific clinical considerations to undertake. Principally, a thorough evaluation of posture by a physiotherapist is essential to suggest suitable seating options (Galea et al., 2020). Postural assessment should be comprehensive, considering factors such as tone, range of motion, functional abilities, and environmental factors. Also, tailored seating, addressing trunk, pelvic, and head support, has the potential to enhance Matthew's posture, lower the risk of aspiration, and aid in feeding. Secondly, physiotherapists need to collaborate closely with occupational therapists and seating specialists to ensure Matthew receives the best seating arrangements both at home and at the day center (Law et al., 2016). Generally, effective postural management requires collaboration among healthcare professionals, including physiotherapists, occupational therapists, orthoptists, and seating specialists. A multidisciplinary approach ensures holistic care and optimal outcomes for CP patients. The final clinical consideration should be regular monitoring and adjustments. CP is a dynamic condition, necessitating ongoing monitoring of posture and seating. Regular reassessment ensures that seating interventions remain effective and adjustments can be made as needed to address changing needs. Hydrotherapy Hydrotherapy, also known as aquatic therapy, is a therapeutic intervention that utilizes the properties of water to address various physical and functional impairments (Law et al., 2016). Matthew currently participates in twice-weekly hydrotherapy sessions. Hydrotherapy offers warmth, buoyancy, and reduced resistance, promoting relaxation, movement control, and muscle strengthening. Matthew could also benefi...
