MR. JUAN Duran Mexican American is visiting the diabetes clinic at the VA , respectful to health care providers and very polite and doesn't like to interrupt or ask to many questions. He has been in the United States for 50 years and is a navy veteran. He was found wondering the hallways and was found by the medical director . he was instructed to start insulin injection for his diabetes and Juan states he doesn't know how to start his insulin. and he doesn't recall receiving an appointment to give instructions. the medical director fitted him in with the education nurse for an unscheduled appointment in the diabetes clinic. Next nurse Jenny is reading his chart and reviews

The delivery of quality nursing care to patients or clients by the different nursing care providers demands the adoption of effective strategies towards applying and implementing the equally diverse nursing theories and practices characterizing the nursing profession. In essence, achieving the desired outcome through quality nursing care requires the development of a comprehensive understanding of the human needs that may influence the efficacy of the nursing care provided. A follow up of Mr. Juan Duran’s case study, a patient visiting the diabetes clinic at the VA, with Neuman’s and Orem’s theories highlight some of the underlying elements characterizing the human needs influencing or determining the achievement of desired outcomes from the delivery of effective nursing care.
Neuman’s Theory
Neuman’s theory or model for the application of nursing practices involves five major categories of human needs. These categories include the psychological, physiological, socio-cultural, developmental, and spiritual needs of the patient or client CITATION Ahm17 \l 1033 (Ahmadi, 2017). An evaluation of Mr. Juan Duran’s case study using the Neuman systems model features a physiological problem of diabetes to which he has to start taking his insulin regimen. The patient also appears to be psychologically unwell and admits to forgetting a previously scheduled appointment to receive instructions on how to start his insulin. The patient also appears to be disoriented, as witnessed by the medical director, who found him wandering the hallways.
