Finding Out the Rationale for the Change of Policy and Discussing It
Nurse Keesha has worked at a community Hospital for 12 years. She was shocked to hear that a new policy going into effect next week includes mandatory delegation of all vital signs to the certified nurse assistants (CNAs), who are to perform them 3 times on each patient each shift. She knows the CNAs with whom she works are already challenged to complete the tasks they have each shift. She thinks vital signs performance on each patient three times each shift is unrealistic if the CNAs are to keep up their other duties.
Keesha knows that if the CNAs attempt to follow the new policy, other tasks will not be performed. Keesha thinks that checking vital signs once per shift is sufficient for most patients unless they are having problems. That is how often the nurses had been required to check vital signs when it was their task.

1.Keesha could find out the rationale for the change in policy by speaking with her manager or hospital administration. Additionally, she could look for any documentation about the policy change that might explain its reasoning. The concept of "Evidence-Based Practice/Spirit of Inquiry" from the worksheet QSEN/NLN competencies would best support Keesha's actions. By inquiring about the rationale for the policy change, Keesha is attempting to understand the problem at hand to make an informed decision about how to respond. This concept emphasizes the importance of problem-solving in the professional realm, which Keesha does by asking questions and gathering information (Giddens, 2017). Keesha's actions demonstrate that she is thinking critically about the policy change and its implications rather than simply accepting it at face value.
2. Keesha should consider discussing the new policy with several different individuals to gain further understanding and potentially advocate for change. First, she may want to speak with her direct supervisor or manager to clarify the reasoning behind the new policy and express her concerns. Suppose Keesha feels that her problems need to be adequately addressed. In that case, she may wish to bring the issue to the hospital administrations or nursing leadership team's attention. Additionally, Keesha could discuss the policy with her fellow nurses to gauge their perspectives and determine if they share her concerns. If they do, they may be able to work together to present a unified front in advocating for a change to the policy. Lastly, Keesha could speak with the CNAs tasked with the new responsibility to better understand their concerns and work collaboratively to address potential challenges.
3. One BSN essential that could be relevant to this situation is Essential VI: Interprofessional Communication and Collaboration for Improving Patient Health Outcomes. In this case, Keesha is...
