Assessment, Delivery and Evaluation Paper Student’s Name Institution Course # and Name Professor’s Name Submission Date Introduction Nurses are required to adhere to the ethics of confidentiality and patient privacy in practice (Nursing Midwifery Council, 2018). The chosen pseudonym for this patient is Mr. TA. This essay will discuss how the student demonstrated assessment and safe delivery of nursing practice and an evaluation of self-management practices. First, the paper will explore examples, relevant theories and arguments that demonstrate competency skills in assessment. Secondly, it will compare and discriminate between ideas on self-management and discuss relevant tools used. Further, the essay will also recognize the laws, policies, and guidelines the student followed to achieve these objectives and the assistance offered by various stakeholders. In this case study, Mr. TA is a 55-year-old man who came to the hospital because he had been experiencing chronic hoarseness and difficulty swallowing. He has a history of being diagnosed with gastroesophageal reflux disease (GERD), hypertension, and mild anxiety concerning his health status based on recent psychological evaluation results. Significant events include multiple episodes of aspiration pneumonia over the past year, likely secondary to his swallowing difficulties. The main issue identified is a suspected laryngeal pathology. Structures crucial for vocalisation and swallowing include the larynx, oesophagus, and associated organs. Mr. TA’s co-morbidities of GERD and hypertension complicate his condition. Mr. TA is also divorced, lives alone and is a school teacher. Competency in Assessment Nursing education teaches student nurses the anatomy and physiology of the larynx (Lungova & Thibeault, 2020). The larynx is responsible for voice production and airway protection during swallowing. Knowledge of vocal cord function and the surrounding musculature was used to explain hoarseness in speech (Boggiano et al., 2022). Laryngeal pathology also complicates recovery from GERD, hence slowing recovery in affected individuals (Brar et al., 2022). Moreover, the student nurse was involved in assessments, physical examinations and treatment planning for Mr. TA (NHS, n.d.). The assessment conducted included an upper endoscopy and barium swallow test. Upper endoscopy assessment showed the esophagus inflammation. The barium swallow test showed inadequate gastric emptying and reflux. The student nurse also ensured that Mr. TA's privacy was respected and that all his details remained confidential, as outlined in the Common Law Duty of Confidentiality (NHS, 2024). The biopsychosocial model for health formed the basis for assessing Mr. TA when he arrived at the healthcare facility (Williamson, 2022). By recognising the interplay of Mr. TA’s physical symptoms, psychological state and social circumstances, the student nurse gained a better understanding of the expected clinical management of the patient (Engle et al., 2021). Mr. TA was referred to a social worker who would monitor him closely during home-based care. Under supervision, referrals to social workers were made because the student nurse could not work independently. In order to comprehend the patient’s state, choices, and expectations, it is necessary for a nurse to take into account the biopsychosocial approach to care delivery. Real-time images of laryngeal cancer signs were provided by laryngoscopy, while computed tomography gave detailed cross-sectional views of the throat. Informed decisions related to Peplau’s theory on interpersonal relationships should be used in relation to client’s health where their autonomy is respected and supported (Fathidokht et al., 2024). In order to augment healing, the student nurse employed recommendations about drug adherence depending on the perspectives of the client and suggested necessary adjustments in life style (NHS England, n.d.). Self-Management in Practice Based on Mukhalalati et al. (2022), Bandura's Social Learning Theory states that self-efficacy, as well as continual learning, is fundamental to professional development. The student was exposed to current research findings on the management of GERD comorbid with laryngeal pathology. In addition, in this situation, the student nurse used proactive problem-solving...
