Introduction
One of the primary issues impacting American community health care outcomes is the medical mistrust and lack of access assurance within most healthcare facilities. Historically, the healthcare services access in the U.S. have been deplorable for people of color, minorities, and other challenged groups. The issues range from low reassurance and insurance of treatment where patients express mistrust and anger. In this vein, the INOVA Fairfax facility was linked to several health differences based on race, gender, ethnicity, education, ZIP code, and income. The minorities lacked reassurance in medication and insurance since they hardly participated in the community-level decision-making. Accordingly, it is well-established that life expectancy varied from one neighborhood to another. This is because where people reside is affected by their educational opportunities, status, economic background, and overall quality of life and health. INOVA’s primary goal is to offer equal care to a large community in Fairfax County, but some residents feel the existence of healthcare access gaps. Medical reforms have been championed, but the the structurally marginalized communities feel surrounded by a lack of progress and reassurance. Many claim they should get opportunities to explore the ‘top-notch’ services offered by the facility in the area without barriers.

In this context, the facility should work based on community-based innovation and offer culturally-appropriate care while concerned with social determinants of healthcare access. In the recent past, the facility has been focusing on behavioral health integration to enhance neighborhood access to trauma and mental disorders in minor neighborhoods in Fairfax County.

For goal one, the strategic objectives include enhancing affordability, high-quality enrollment, choice in community health care coverage, and integrating behavioral health to reach minor families with mental and trauma issues.

To effectively implement an action plan that caters to the needs of the overall community, Harrison (2016a) proposes that the healthcare facility needs to stabilize community insurance marketplaces and adopt ACA (Affordable Care Act) reforms. In order to override the existing conditions, INOVA should discourage the sale of healthcare insurance to specific groups. In addition, this effort is critical to reducing health care disparities within the community and the nation at large.

The priority in measuring these goals is identifying the key personnel from the facility and community to minimize resistance to the change model. The community is vital to ensure that the action plan is patient-driven, realistic, and specific (Harrison, 2016b). Another measurement is to improve factors that impact individuals’ access to health care services, including their behavioral inclinations and choice, community-related issues such as unemployment, poverty, inadequate housing, poor educational outcomes, poor means of transportation, and interpersonal violence. Other factors measuring access to health care facilities include decaying neighborhoods and ongoing or historical interplay of structures, norms, and policies, shaping individuals’ lives.
