The reference for this article is mainly to describe the transition of care from hospital to home for patients with Hypertension and to discuss the method practiced in communicating with the patients in time of discharge. Hypertension is a health problem which is gradually affecting one in three adults in the United States, whereby according to research about 50% to70% of the patients admitted in hospital have hypertension. However, patients do experience a transition of care at the time of discharge as they move from the hospital to home, as they are likely to experience unpropitious events in relation to the medication such as missing or changed medications hence putting the patient at risk. Hence the clinicians has to ensure that they pass the communication in a more clear and understandable way. ‘‘Findings indicate that a long-term patient with Hypertension need more unified measurement tools in order to evaluate continuity of care’’, (Ye & Ting, 2016).
Nursing practice is relevant when it comes to transition of care from hospital to home for patients with Hypertension in the sense that, there are hospital knowledgeable clinicians who are in the best position to giving guidelines on how to manage hypertension for any hospitalized patients and to be able to address any necessary information between the patient and the family in time of discharge.
