School accommodation for work

I was denied five times for a hardship transfer application to a part-time shift and was again denied for a school accommodation to adjust six hours of my regular schedule. Initially, my request was rejected due to insufficient documentation, but even after submitting all required documents, I faced subsequent denials, citing reasons such as the inability to facilitate the transfer. Furthermore, after submitting my school accommodation request, it was denied on the grounds that it does not interfere with my work schedule. However, I am currently enrolled in four courses totaling 15 credits, for which I have submitted documentation related to a disorder necessitating accommodation. Despite reaching out to the Employee Relations Committee (ERC) multiple times, no assistance was provided. Similarly, when contacting site HR members, I was advised to resubmit my request, which I did, yet it was met with multiple denials. The HR site manager indicated they would investigate my case and provide a response by Friday, May 17th, 2024, but I have not received any communication via email or in-person. Additionally, my attempts to seek assistance from my Operation Manager proved futile, as they stated they have no control over the matter. I am beginning to feel stressed and fear I may spiral into depression, similar to my experience last semester when my school accommodation request was denied, leading to failing grades and depression. Despite reporting feeling unwell, I received a performance write-up for my poor academic performance. Therefore, I am reaching out to the CEO for assistance in resolving this matter. Please help me write a professional school accommodation to the chairman. thank you.
